目的 探讨十色流式细胞术在多发性骨髓瘤(multiple myeloma, MM)免疫表型中的应用价值.方法 采用十色荧光抗体组合,CD45/SSC配合CD38/CD138设门,对106 例MM患者治疗前后骨髓浆细胞表面CD19、CD56、CD117、CD81、CD137L及胞质轻链cKappa、cLambda进行检测;并采用以上组合对20 名健康志愿者标本进行检测并进行比较分析.结果 十色流式细胞术在MM患者治疗前骨髓中克隆性浆细胞(cPCs)的阳性率为92.6%.与正常浆细胞不同,MM患者cPCs异常表达CD19-(97.4%)、CD56+(89.6%)、CD81±(60.4%)、CD117+(18.6%)、CD137L+(12.6%),且各抗原的表达在正常浆细胞与MM患者之间差异具有统计学意义.MM患者治疗后骨髓中cPCs的抗原表达与治疗前差异有统计学意义.结论 十色流式细胞术有助于检出cPCs,更好地区分良恶性浆细胞,辅助临床对MM患者进行诊断,为其治疗及治疗后监测提供依据.“,”Objective To explore the value of decolor flow cytometry in detecting the immunophenotype of multiple myeloma (MM). Methods Using a combination of ten color fluorescent antibodies, CD45/SSC with CD38/CD138 set the door, 106 cases of MM before and after treatment, bone marrow plasma cell surface CD19, CD56, CD117, CD81, CD137L, and cytoplasmic light chain cKappa, cLambda were tested. The samples of 20 healthy volunteers were tested and compared. Results The positive rate of clonal plasma cells (cPCs) in bone marrow and peripheral blood in patients with MM was 92.6%. Unlike normal plasma cells, abnormal expression of CD19- (97.4%),CD56+ (89.6%), CD81± (60.4%), CD117+ (18.6%), CD137L+ (12.6%) in patients with MM. There was significant difference in the expression of antigens between normal and patients with MM. Antigen expression of cPCs in bone marrow after treatment with MM was different from that before treatment, and it was statistically significant. Conclusion Ten color flow cytometry is helpful to detect the cPCs, to divide the good and bad plasma cells better, to assist the diagnosis of MM patients, and to provide evidence for its treatment and after-treatment monitoring.
