目的观察中药健肾颗粒剂对糖尿病肾病(DN)大鼠肾脏组织内皮素-1(ET-1)mRNA表达及肾功能的影响。方法雄性Wistar大鼠40只,采用5/6肾切除加尾静脉注射链脲佐菌素(STZ)的方法复制大鼠DN动物模型,并随机分为健肾颗粒剂、苯那普利、保肾康、病理对照4组,每组10只,另设正常对照组10只。采用生化法检测血肌酐(SCr)、尿素氮(BUN);肾脏组织标本,经染色后用光学及电子显微镜观察肾脏组织病理结构;用PCR方法检测肾脏组织中ET-1mRNA。结果健肾颗粒剂组大鼠SCr、BUN水平低于苯那普利、保肾康及病理对照组(P<0.05或P<0.01);健肾颗粒剂组肾脏组织中ET-1mRNA表达明显低于其他3组(P<0.05或P<0.01)。光镜下肾脏组织病理结构示健肾颗粒剂组,肾小球体积,球囊粘连程度,肾小球系膜细胞增生程度,系膜基质等情况均较其他3组有明显改善;而电镜下肾小球上皮细胞增生、肿胀、足突融合、低矮、扁平、以及假绒毛样变,系膜细胞增生,基质增生,毛细血管扩张、细胞堆积等病理变化也均较其他3组有明显减轻。健肾颗粒剂降低DN大鼠血糖、尿蛋白及血脂的作用明显优于苯那普利、保肾康及病理对照组(P<0.05或P<0.01)。结论健肾颗粒剂可明显改善DN大鼠肾功能,并有降血糖、调整血脂,下调DN大鼠肾脏ET-1mRNA的作用,同时改善肾脏的病理组织结构。
Objective To observe the effect of Jianshen Granules on renal endothelin-1 (ET-1) mRNA expression and renal function in rats with diabetic nephropathy (DN). METHODS: Forty male Wistar rats were randomly divided into Jianshen granules, Benazepril, and Proline, using a 5/6 nephrectomy plus tail vein injection of streptozotocin (STZ). Kidney-Kang and pathology were compared in 4 groups, 10 in each group, and 10 in the normal control group. Serum creatinine (SCr) and urea nitrogen (BUN) were detected by biochemical methods; kidney tissue samples were stained, and the pathological structure of kidney tissues was observed by optical and electron microscopy; and ET-1 mRNA in kidney tissues was detected by PCR. Results The serum levels of SCr and BUN in the Jianshen Granule group were lower than those in the Benazepril, Baoshenkang and pathological groups (P<0.05 or P<0.01). The expression of ET-1 mRNA in the kidney tissue of the Jianshen Granule group was significantly lower. In the other 3 groups (P<0.05 or P<0.01). The histopathological structure of renal tissue under light microscope showed that Jian Ren Granule group, glomerular volume, degree of balloon adhesion, glomerular mesangial cell proliferation, and mesangial matrix were significantly improved compared with the other three groups; Glomerular epithelial cell proliferation, swelling, fusion of foot processes, low, flat, and false villous changes, mesangial cell proliferation, matrix proliferation, telangiectasia, cell accumulation and other pathological changes are also significantly reduced compared with other three groups . The effect of Jianshen Granules on reducing blood glucose, urine protein and blood lipid in DN rats was significantly better than that of Benazepril, Baoshenkang and pathological control group (P<0.05 or P<0.01). Conclusion Jianshen Granules can significantly improve the renal function of DN rats, reduce blood sugar, regulate blood lipids, and down-regulate the effects of ET-1 mRNA in kidneys of DN rats, as well as improve the pathological structure of kidneys.
