目的探讨18F-脱氧葡萄糖正电子发射计算机体层成像(18 F-FDG PET/CT)全身显像诊断结直肠癌术后复发和转移的价值。方法结直肠癌术后患者23例行28人次18 F-FDG PET/CT全身显像,半定量分析病灶PET显像的标准摄取值,并计算延迟显像双时相扫描储留指数(RI)。随访12-36个月,评价PET、低剂量平扫CT和PET/CT显像诊断结直肠癌术后复发和转移的灵敏度、特异性、准确性。结果 PET/CT显像中,恶性病灶的SUVmax值为7.15±3.67,RI值为(46.1±22.2)%。PET/CT显像诊断复发和转移的灵敏度、特异性、准确性、阳性预测值、阴性预测值分别为97.6%、100.0%、97.8%、100.0%、80.0%。PET和低剂量平扫CT诊断复发和转移的灵敏度、特异性、准确性、阳性预测值、阴性预测值分别为97.6%、75.0%、95.6%、97.6%、75.0%和90.0%、60.0%、86.7%、94.7%、42.9%。结论 18F-FDG PET/CT全身显像诊断结直肠癌术后复发和转移有较高准确性,可为肝转移和腹腔早期局部转移的再次手术提供方便。
Objective To investigate the value of 18F-FDG PET/CT whole body scintigraphy in the diagnosis of recurrence and metastasis of colorectal cancer. Methods Twenty-three patients with postoperative colorectal cancer underwent 18 F-FDG PET/CT whole-body imaging with 28 patients. The standard uptake value of lesion PET imaging was analyzed semi-quantitatively, and the delayed imaging dual-phase scan retention index (RI) was calculated. . After 12-36 months of follow-up, the sensitivity, specificity, and accuracy of PET, low-dose unenhanced CT, and PET/CT imaging in the diagnosis of recurrence and metastasis of colorectal cancer were evaluated. Results In PET/CT imaging, the SUVmax of malignant lesions was 7.15±3.67, and the RI was (46.1±22.2)%. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of PET/CT imaging for recurrence and metastasis were 97.6%, 100.0%, 97.8%, 100.0%, and 80.0%, respectively. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of PET and low-dose unenhanced CT in the diagnosis of recurrence and metastasis were 97.6%, 75.0%, 95.6%, 97.6%, 75.0%, and 90.0%, 60.0%, respectively. 86.7%, 94.7%, 42.9%. Conclusion 18F-FDG PET/CT whole body imaging has high accuracy in the diagnosis of recurrence and metastasis of colorectal cancer, and it can provide convenience for reoperation of liver metastasis and early local metastasis of the abdominal cavity.
