患儿女,7个月,入院日期:1984年12月14日,因口唇及肢端青紫2个月,呼吸困难1个月入院。患儿自生后5个月渐出现口唇及四肢末端发青发凉。1个月来吃奶时呼吸困难,常多次被(?)停止吸吮。患儿为第二胎。一年前,第一胎女婴于生后4个月出现与本例同样症状,生后6个月死亡。当地诊断“心脏病”。患儿父母均健康。体检:体温36.5℃,脉搏140次,呼吸52次,体重6.5kg。发育营养欠佳,眼睑浮肿。口唇、双手足指趾轻度发绀,哭声微弱。心前区轻度隆起,心尖搏动消失,心界向两侧扩大。心尖第一心音低钝、律整,无杂音。两肺无罗音。肝肋下3cm,剑突下4cm,质中等边缘钝。下肢无指凹性浮肿。胸部 X 线照片示:心脏呈普大型,以向左下
