目的探讨产后出血量的评估方法及影响产后出血的相关因素,旨在为妇产科临床治疗提供可靠的参考数据。方法选取我院妇产科2012年3月—2013年7月收治的产后出血患者128例,随机将患者分为对照组及观察组各64例,对照组采用主观测定法(目测法)来评估产后出血量,观察组则采用客观测定法(称重法、容积法、羊水压积测定法),同时观察影响患者产后出血的相关因素。结果观察组评估产后出血量为439.5ml,对照组为210.9ml,观察组与实际出血量符合度为100%,对照组则少于约一半,两组比较差异具有显著统计学意义(P<0.05)。生理病理、心理、社会环境等诸多方面均与产后出血有一定的关系,其中宫缩乏力、胎盘粘连、心理状态差是导致产后出血的主要因素。结论在临床的实际应用中,客观测定法对于产后出血量的评估准确率较高,可为临床正确评估产后出血量提供可靠的参考价值,值得临床上推广应用。充分认识产后出血的危险因素,可以预测产后出血量,以便及时采取措施进行治疗。
Objective To explore the assessment of postpartum hemorrhage and related factors affecting postpartum hemorrhage, which aims to provide reliable reference data for the clinical treatment of obstetrics and gynecology. Methods 128 cases of postpartum hemorrhage admitted to our department from March 2012 to July 2013 were randomly divided into control group and observation group with 64 cases each. The control group was evaluated by subjective evaluation (visual inspection) The amount of postpartum hemorrhage, the observation group was the use of objective determination (weighing method, volume method, amniotic fluid pressure test), while observing the impact of postpartum hemorrhage in patients with related factors. Results In observation group, the postpartum hemorrhage volume was 439.5ml and the control group was 210.9ml. The coincidence degree between the observation group and the actual bleeding amount was 100%, while the control group was less than about half. The difference between the two groups was statistically significant (P <0.05 ). Physiological and pathological, psychological, social environment and many other aspects of postpartum hemorrhage have a certain relationship, including uterine inertia, placental adhesion, poor psychological status is the main factor leading to postpartum hemorrhage. Conclusion In clinical practice, objective assessment method for postpartum hemorrhage assessment of the high accuracy rate for clinical correct assessment of postpartum hemorrhage provide a reliable reference value, it is worth promoting the clinical application. Fully aware of the risk factors for postpartum hemorrhage, postpartum hemorrhage can be predicted, in order to take timely measures for treatment.
