目的探讨阿尔茨海默病(AD)患者磁共振成像(MRI)海马体积、MRI弥散张量成像(DTI)参数与其认知功能的相关性。方法研究对象包括30例临床诊断为AD的患者组和30例认知功能正常老年人的对照组。利用MRI、DTI对双侧海马区进行扫描,比较2组间海马体积、海马区域部分各向异性(FA)值及表观弥散系数(ADC)之间是否具有差异,评价以上参数与认知功能之间的关系。结果 AD组与对照组MMSE、ADL评分、左右侧海马体积、海马区域DTI参数比较,差异均有统计学意义(P<0.01)。左侧、右侧海马体积与MMSE、ADL均呈正相关性(r值分别为0.892、0.721、0.836、0.749,P<0.01)。AD患者海马区域的FA值与其MMSE值呈正相关(r=0.629,P<0.05);AD患者海马区域的ADC值与其MMSE值呈负相关(r=-0.491,P<0.05)。结论 AD患者的海马区MRI表现与认知功能有相关性。
Objective To investigate the relationship between magnetic resonance imaging (MRI) hippocampal volume, MRI diffusion tensor imaging (DTI) parameters and cognitive function in patients with Alzheimer’s disease (AD). Methods The subjects included 30 patients with clinically diagnosed AD and 30 controls with normal cognitive function. MRI and DTI were used to scan bilateral hippocampal regions. The hippocampal volume, the partial anisotropy (FA) value and the apparent diffusion coefficient (ADC) of the hippocampus were compared between the two groups. Results There were significant differences in MMSE, ADL score, right and left hippocampal volume and DTI parameters in hippocampus between AD group and control group (P <0.01). Left and right hippocampal volume and MMSE, ADL were positively correlated (r values were 0.892,0.721,0.836,0.749, P <0.01). The FA value of hippocampus in AD patients was positively correlated with MMSE (r = 0.629, P <0.05). The ADC value of hippocampus in AD patients was negatively correlated with MMSE (r = -0.491, P <0.05). Conclusion MRI findings of hippocampus in AD patients are related to cognitive function.
