[目的]探讨阿茨海默病(AD)患者睡眠结构异常与认知功能的相关性.[方法]选取 2016 年 1 月至2018 年 12 月在本院就诊的 AD患者 100 例,其中合并睡眠结构异常患者 50 例(观察组),单纯 AD患者 50 例(对照组).比较两组患者睡眠参数指标变化情况;采用蒙特利尔认知评估量表(MoCA)和简易智力状况检查量表(MMSE)评价患者的认知功能;采用 Spearman相关分析方法分析AD患者睡眠结构异常和认知功能的相关性.[结果]两组醒觉时间、NREM 2 期、NREM 1+2 期所占比例比较差异无显著性(P >0.05);与对照组相比,观察组 NREM 1 期、NREM期所占比例明显升高(P <0.05),NREM 3+4 期、REM 期期、MMSE评分、MoCA评分明显降低(P <0.05).观察组患者MMSE评分与NREM 3+4期、夜间最低动脉血氧饱和度(SaO2 )、夜间平均SaO2 呈正相关(P <0.05),与NREM 1+2 期、ODI、AHI呈负相关(P <0.05),与REM期、微觉醒指数没有线性关系.MoCA评分与夜间平均 SaO2 和夜间最低 SaO2 呈正相关(P <0.05),与 INREM 1+2 期、血氧饱和度下降指数(ODI)、呼吸暂停通气指数(AHI)呈负相关(P <0.05).MoCA评分与微觉醒指数、ODI呈负相关(P <0.05),与 REM期呈正相关(P <0.05),与夜间最低SaO2 、夜间平均SaO2 、NREM 1+2 期、NREM 3+4期、AHI无线性相关(P >0.05).[结论]老年痴呆患者睡眠结构出现异常,认知功能下降,其睡眠结构变化与认知功能可能存在相关性.“,”[Objective]To investigate the relationship between sleep structure abnormalities and cognitive function in patients with Alzheime'rs disease.[Methods]A total of 100 patients with Alzheime'rs disease who were treated in our hospital from January 2016 to December 2018 were enrolled,including 50 patients with senile dementia with sleep structural abnormalities (the observation group)and 50 patients with senile dementia without sleep structural abnormalities (the control group).The changes in sleep parameters were compared between the two groups,and the cognitive function was assessed by using both Montreal Cognitive Assessment Scale (MoCA)and Mini Mental State Examination (MMSE).The Spearman correlation was used to analyze the correlation between sleep structural abnormalities and cognitive function in patients with Alzheime'r s disease.[Results]There was no significant difference in the proportion of wakeup time,NREM 2 phase,and NREM 1+2 phase between the two groups (P>0.05).Compared to the control group,the proportion of NREM 1 phase and NREM phase in the observation group was significantly increased (P <0.05)and the proportion of the NREM 3+4 phase,REM, MMSE,and MoCA scores were significantly lower (P <0.05)in the observation group.The MMSE score of the observation group was positively correlated with NREM 3+ 4 phase,the nighttime minimum SaO2,and the average SaO2 at night (P<0.05),however,the MMSE score of the observation group was negatively correlated with NREM phase 1 + 2,ODI,AHI (P <0.05).There was no linear correlation of MMSE with REM phase and mi croawakening index (P>0.05).On the other hand,the MoCA score was positively correlated with the nighttime minimum SaO2 and nighttime average SaO2 (P<0.05),and negatively correlated with INREM 1 + 2 phase,oxygen saturation decline index (ODI)and apnea ventilation index (AHI)(P <0.05).The MoCA score was negatively correlated with microawakening index and ODI (P <0.05),and positively correlated with the REM phase (P <0.05),and no linearly correlated with the nighttime minimum SaO2 ,the average SaO2 at night,the NREM
