我们采用以阿是穴为主穴,合谷穴为配穴的激光镇痛拔牙法,取得良好的效果。 采用由单芯光导纤维输出的He-Ne激光,波长632.8nm,光纤端输出功率〉10mW。根据针炙阿是穴和拔牙浸润麻醉原理,在操作时,先将光纤输出头伸入口内,尽可能垂直地直接压在患牙腭、舌面各根根端龈部,光照时间每根4~6min,再将光纤输出头尽可能垂直地直接压在患牙颊、唇面各根根端龈部,光照时间不变。然后,将光纤输出头移照合谷穴2min,开始拔牙,激光对合谷穴的照射持续到手术结束。在操作前,为了避免光纤输出端被口腔污物污染而减弱激光功率,并防止患者间的交叉感染,必须先用75%酒精消毒。照射时应注意光纤输出头要轻压龈部,否则,将引起患者牙龈的不必要的疼痛。
