[目的]探讨铁矿井下作业人员动脉顺应性的变化,为进一步制定相应的预防与控制措施提供科学依据。[方法]使用动脉脉搏波形分析仪测量219名南京市某钢铁公司井下作业人员(观察组)的动脉功能:大动脉弹性指数(C1)、小动脉弹性指数(C2)、收缩压(SBP)、舒张压(DBP)、脉压差(PP);用声级计测量井下噪声强度;用听力测定仪测定工人左右耳听力;另取50名南京市不接触职业性有害因素工作人员作为对照组。[结果]铁矿井下噪声强度为107.2~117.2dB(A),作业人员听力下降率为61.5 c/o。观察组的C1、C2均明显低于对照组(P<0.05),SBP、DBP则明显高于对照组(P<0.01)。听力损失组SBP、DBP、PP均显著高于对照组和听力正常组(P<0.05),而听力损失组的C1、C2则要明显低于对照组。单耳听力下降组、双耳听力下降组的C1、C2均明显低于对照组(P<0.05)。分别以C1、C2为因变量,年龄、血压为自变量进行回归分析,发现血压对C1、C2影响很大,其中SBP主要影响C1,DBP主要影响C2。[结论]铁矿井下作业人员的动脉顺应性降低,血压水平增高,考虑主要与作业环境的强噪声有关。
[Objective] The research aimed to discuss the change of arterial compliance of workers in iron mines and provide a scientific basis for further developing corresponding prevention and control measures. [Method] Arterial pulse wave shape analyzer was used to measure the arterial function of 219 underground workers (observation group) of a steel company in Nanjing city: arterial index (C1), arterial elasticity index (C2), systolic blood pressure (SBP) Pressure (DBP) and pulse pressure difference (PP). The downhole noise intensity was measured with a sound level meter. The left and right ear hearing of the workers was measured with a hearing tester. Another 50 workers in Nanjing who were not exposed to occupational harmful factors were taken as the control group. [Result] The underground noise intensity of iron mine was 107.2 ~ 117.2dB (A), and the hearing worker’s hearing loss rate was 61.5 c / o. C1 and C2 in the observation group were significantly lower than those in the control group (P <0.05), and SBP and DBP were significantly higher in the observation group than those in the control group (P <0.01). The SBP, DBP and PP in the hearing loss group were significantly higher than those in the control group and the hearing normal group (P <0.05), while those in the hearing loss group were significantly lower than those in the control group. C1 and C2 of both hearing loss group and hearing loss group were significantly lower than those of the control group (P <0.05). Respectively with C1, C2 as dependent variables, age, blood pressure as an independent variable regression analysis found that blood pressure on C1, C2 a great impact, including SBP mainly affect C1, DBP mainly affect C2. [Conclusion] The arterial compliance of iron mine workers is lower and the blood pressure level is higher, which is mainly related to the strong noise of working environment.
