本文利用计算流体力学方法来探讨肝右静脉与下腔静脉之间夹角的变化对布-加综合征下腔静脉隔膜发生机制的影响。运用Mimics软件对临床磁共振成像(MRI)血管造影下腔静脉、肝右静脉、肝中静脉、肝左静脉断层图像进行三维数字化重建,将重建模型导入3DMAX软件分别构建肝右静脉与下腔静脉之间夹角为30°、60°、90°和120°的模型。结合相关临床参数计算得到模型壁面剪切应力场、壁面压强场、速度场等分布特征。应用SPSS统计学软件分析,结果表明下腔静脉与肝右静脉的夹角不同,对应的壁面压强及壁面剪切应力存在显著差异且具有统计学意义(P<0.05),下腔静脉隔膜阻塞的形成与夹角的变化有一定的相关性。
In this paper, computational fluid dynamics is used to investigate the effect of the angle between the right hepatic vein and inferior vena cava on the mechanism of inferior vena cava membrane in Budd-Chiari syndrome. Three-dimensional digital reconstruction of inferior vena cava, right hepatic vein, middle hepatic vein and left hepatic venous thrombosis in clinical magnetic resonance angiography (MRI) was performed using Mimics software. The reconstruction model was imported into 3DMAX software to construct the right hepatic vein and inferior vena cava The included angle is between 30 °, 60 °, 90 ° and 120 °. The distribution of shear stress field, wall pressure field and velocity field on the model wall were calculated with the related clinical parameters. SPSS statistical software analysis showed that the angle between inferior vena cava and right hepatic vein was different, the corresponding wall pressure and wall shear stress were significantly different (P <0.05), and inferior vena cava septal obstruction Formation and angle changes have some relevance.
