目的:探求治疗急性有机磷农药中毒中间综合征(IMS)的有效方法。方法:113例急性有机磷农药中毒IMS患者随机分为治疗组、对照A组和对照B组。治疗组51例在综合治疗基础上除用氯磷定外,予血液灌流和口服中药“排毒汤”。对照A组32例在综合治疗基础上单用氯磷定。对照B组在综合治疗基础上采用氯磷定和血液灌流治疗。结果:治疗组治愈率为96.08%,显著优于对照A组(21.88%,P<0.05)和对照B组(73.33%,P<0.05);存活率为100.00%,显著优于对照A组(31.25%,P<0.005)。治疗组IMS消失时间为(22.14±5.77)小时,亦明显短于对照A组和对照B组〔(328.75±21.99)小时和(38.51±6.23)小时,P均<0.001〕。治疗组无一例发生呼吸衰竭。治疗组血液灌流后阿托品用量明显降低(P<0.001),胆碱酯酶无明显变化(P>0.05)。结论:血液灌流可迅速清除体内有机磷农药、毒性代谢产物以及体内蓄积的乙酰胆碱;中药“排毒汤”可清除胃肠道残留的有机磷农药,防止毒物的继续吸收,二者配合治疗急性有机磷农药中毒IMS疗效显著。
Objective: To explore an effective method for the treatment of acute organophosphorus pesticide poisoning intermediate syndrome (IMS). Methods: One hundred and thirty-one patients with acute organophosphorus pesticide poisoning were randomly divided into treatment group, control group and control group. In the treatment group, 51 cases were treated with hemoperfusion and oral Chinese medicine “detox soup” on the basis of comprehensive treatment except chlorotoptin. Control group A 32 cases on the basis of comprehensive treatment alone chlorpromazine. The control group B was treated with chlorophosphate and hemoperfusion on the basis of comprehensive treatment. Results: The cure rate of the treatment group was 96.08%, which was significantly better than that of the control group A (21.88%, P <0.05) and the control group B (73.33%, P <0.05). The survival rate was 100.00%, significantly better than the control group A (31.25%, P <0.005). IMS disappeared in the treatment group (22.14 ± 5.77) hours, which was also significantly shorter than those in the control group A and control group [(328.75 ± 21.99) hours and (38.51 ± 6.23) hours , P <0.001]. No case of respiratory failure occurred in the treatment group. The amount of atropine in the treatment group decreased significantly after hemoperfusion (P <0.001), but there was no significant change in cholinesterase (P> 0.05). Conclusion: Hemoperfusion can quickly remove organophosphorus pesticides, toxic metabolites and the accumulation of acetylcholine in vivo. Traditional Chinese medicine Detox Soup can remove organophosphorus pesticide residues in the gastrointestinal tract to prevent the continued absorption of toxic substances.
