目的:观察异丙酚对安氟醚最低肺泡有效浓度(MAC)及手术切皮无应激反应的MAC(MAC-BAR)的影响。方法:ASAI-Ⅱ级择期手术病人30例,随机分为两组,以手术切皮为疼痛刺激方法。异丙酚用3mgkg-1的诱导剂量加53.8μgkg-1min-1连续输注。结果:测得安氟醚MAC为1.609Vol%,MAC-BAR为1.736Vol%,与经典值相对照,异丙酚对安氟醚MAC没有明显影响,但能降低MAC-BAR35.5%。结论:异丙酚镇痛作用很弱或没有镇痛作用,异丙酚应用后不必用高浓度吸入麻醉药即可阻断循环与内分泌反应,用MAC-BAR作麻醉强度指标,能显著减少或基本消除手术中的心血管及神经内分泌应激反应。
Objective: To investigate the effect of propofol on the minimum alveolar concentration (MAC) and MAC-BAR in surgical incision. Methods: Thirty ASAI-Ⅱ patients undergoing elective surgery were randomly divided into two groups. Surgical skin incision was used as pain stimulus. Propofol was continuously infused with an induction dose of 3 mg kg-1 plus 53.8 μg kg-1 min-1. Results: The enflurane MAC was 1.609Vol% and the MAC-BAR was 1.736Vol%. Compared with the classical value, propofol did not affect the enflurane MAC, but could reduce the MAC-BAR 35.5% . CONCLUSION: The analgesic effect of propofol is weak or no analgesic effect. Propofol can block the circulatory and endocrine reactions without inhalation of high concentrations of anesthetics. The use of MAC-BAR as anesthetic strength index can significantly reduce or The basic elimination of cardiovascular and neuroendocrine stress during surgery.
