目的观察同步放化疗治疗局部晚期鼻咽癌的近期疗效和毒副反应。方法 30例初治局部晚期鼻咽低分化鳞癌患者,随机分为单纯放疗组15例,同步放化组15例。两组放疗方法和剂量相同。同步组自放疗第一天起开始应用顺铂30mg/(m2·周),并与放疗同步进行。结果同步组和单放组的有效率分别为93.3%和86.7%,两组比较无统计学差异(P>0.05),但同步组CR率为60%明显高于单放组46.7%,且有显著统计学差异(P<0.05)。两组均出现不同程度的口腔黏膜炎,但同步组Ⅲ度口腔黏膜炎的发生率明显高于单放组,两组比较有显著统计学差异(P<0.05)。Ⅰ～Ⅱ度骨髓抑制和消化道反应的发生率同步组较单放高,有统计学差异(P<0.05),但对症治疗后,均可好转。结论同步组完全缓解率高,近期疗效肯定,毒副反应患者可以耐受。
Objective To observe the curative effect and toxicity of concurrent chemoradiotherapy in the treatment of locally advanced nasopharyngeal carcinoma. Methods Thirty patients with locally advanced nasopharyngeal poorly differentiated squamous cell carcinoma were randomly divided into radiotherapy alone group (n = 15) and concurrent radiotherapy group (n = 15). Radiotherapy methods and doses of the same two groups. Synchronous group from the first day of radiotherapy began to apply cisplatin 30mg / (m2 · weeks), and with radiotherapy simultaneously. Results The effective rates of the synchronous group and the single radiotherapy group were 93.3% and 86.7%, respectively. There was no significant difference between the two groups (P> 0.05), but the CR rate of the synchronous group was significantly higher than that of the single radiotherapy group (46.7%) Significant statistical difference (P <0.05). There were different degrees of oral mucositis in both groups, but the incidence of oral mucositis in group Ⅲ was significantly higher than that in group Ⅲ. There was significant difference between the two groups (P <0.05). The incidence of Ⅰ ~ Ⅱ myelosuppression and gastrointestinal reaction in the synchronous group was significantly higher than that in the single group (P <0.05), but the symptomatic treatment could be improved. Conclusion The complete remission rate in the synchronous group is high, and the curative effect in the near future is definite. The patients with toxic side effects can tolerate.
