目的分析 CT、 MRI导向丘脑、苍白球毁损治疗帕金森病的疗效。方法使用驹井式和 Leksell脑立体定向仪,运用 CT或 MRI导向,对 12例帕金森病进行毁损术。结果所有病例在术中其震颤全部消失,肌张力较术前明显下降。手术后一周均行头颅 MRI扫描, MRI导向术的误差在 1 mm以内, CT导向术的误差在 3 mm左右。经 6月以上的随访,总有效率达 100%。结论本组研究证实 MRI导向优于 CT导向。术后随访发现,丘脑腹中间核对震颤效果明显,苍白球腹后外侧部对僵直效果明显。
Objective To analyze the curative effect of CT and MRI-guided thalamus and globus pallidus on Parkinson’s disease. Methods Komachi-type and Leksell stereotaxic instrument were used in the study. 12 cases of Parkinson’s disease were destroyed by CT or MRI guidance. Results All cases disappeared during surgery, muscle tension decreased significantly compared with preoperative. The head MRI scan was performed one week after surgery. The error of MRI-guided surgery was less than 1 mm. The error of CT-guided surgery was about 3 mm. After more than 6 months of follow-up, the total effective rate of 100%. Conclusion This study confirmed that MRI guidance is superior to CT guidance. Postoperative follow-up found that the middle of the thalamic ventral tremor significant effect, pale lateral ventral ball on the effect of stiff.
