目的探讨口腔黏膜癌组织缺氧诱导因子-1α(hypoxia-inducible factor 1 alpha,HIF-1α)的表达及其与微血管密度(microvessel density,MVD)、临床病理因素之间的关系。方法采用免疫组化SP法检测30例口腔黏膜癌组织中HIF-lα的表达,用CD34单克隆抗体标记血管内皮细胞,采用Motic数码医学图象分析系统(MoticMed system 6.0A)采集图象,并计数微血管密度。结果①口腔黏膜癌组织中,HIF-lα蛋白高表达;正常口腔黏膜组织中,HIF-lα无表达(P<0.05);②口腔黏膜癌组织的MVD明显高于正常口腔黏膜组织(P<0.05);③HIF-1α蛋白表达与MVD表达呈正相关(P<0.05),与患者年龄、肿瘤浸润深度无关(P>0.05);HIF-1α表达水平随肿瘤临床分期的增高而增加(P<0.05),与肿瘤的组织学分化程度呈负相关;④伴有淋巴结转移者的HIF-lα表达水平明显高于不伴有淋巴结转移者(P<0.05)。结论HIF-1α在口腔黏膜癌组织中高表达,并可促进口腔黏膜癌组织的微血管形成,可能在口腔黏膜癌的发生和发展中起重要作用,HIF-1α可能成为预测口腔黏膜癌侵袭、转移及预后的重要指标。
Objective To investigate the expression of hypoxia-inducible factor-1α (HIF-1α) in oral mucosal carcinoma and its relationship with microvessel density (MVD) and clinicopathological factors. Methods Immunohistochemical SP method was used to detect the expression of HIF-1α in 30 cases of oral mucosal cancerous tissues. The endothelial cells were labeled with CD34 monoclonal antibody and the images were acquired by MoticMed system 6.0A Microvessel density was counted. Results ① The expression of HIF-1α protein in oral mucosa was significantly higher than that in normal oral mucosa (P <0.05), while the expression of HIF-1α was not found in normal oral mucosa (P <0.05). ② The MVD in oral mucosal carcinoma was significantly higher than that in normal oral mucosa (P <0.05). The expression of HIF-1α was positively correlated with MVD (P <0.05), but not with age and the depth of tumor invasion (P> 0.05). The expression of HIF-1α increased with the clinical stage (P < , Which was negatively correlated with the degree of histological differentiation. (4) The expression of HIF-1α in patients with lymph node metastasis was significantly higher than those without lymph node metastasis (P <0.05). Conclusions HIF-1α is overexpressed in oral mucosa and may promote angiogenesis in oral mucosa. It may play an important role in the occurrence and development of oral mucosal carcinomas. HIF-1α may be an indicator of invasion and metastasis of oral mucosa Important prognostic indicators.
