目的对比分析冠脉CT成像技术与冠脉造影技术在冠心病诊断上的应用价值。方法选取我院2014年1月~2015年12月中接收并怀疑为冠心病的97例患者,其均出现心前区不适、胸口痛等临床表现。对所选患者分别采用64排螺旋CT(先行静脉注射造影剂)与冠脉造影检查,观察两种检查方法诊断冠心病的一致性,并比较其敏感性与特异性。结果由冠脉造影检查共诊断出85例冠心病,冠脉CTA检查出84例,两种检查方式同时检查82例冠心病,其中冠脉CTA出现1例漏诊现象;冠脉CTA显示血管病变共276支,血管正常112支,冠脉造影检查显示血管病变275支,正常血管113支,两种检查方法结果高度符合。评估两种检查方式应用价值发现,冠脉造影敏感性、特异性及准确性分别为97.94%、97.94%、98.97%,而冠脉CTA分别为98.97%、96.91%、97.94%,组间差异经统计学处理显示无意义。P>0.05。结论冠脉造影与冠脉CTA在诊断冠心病中均具有较高应用价值,但相比之下,冠脉CTA技术,较为安全可靠,对患者创伤小、风险低,易被临床患者以医师所接受。
Objective To compare and analyze the value of coronary CT angiography and coronary angiography in the diagnosis of coronary heart disease. Methods Ninety-seven patients who were admitted to our hospital from January 2014 to December 2015 and suspected to have coronary heart disease were enrolled in the study. All patients presented with precordial discomfort and chest pain. The selected patients were treated with 64-slice spiral CT (prior intravenous contrast agent) and coronary angiography to observe the consistency of the two methods to diagnose coronary heart disease, and to compare the sensitivity and specificity. Results A total of 85 cases of coronary heart disease were diagnosed by coronary angiography, 84 cases were diagnosed by coronary CTA, and 82 cases were diagnosed by coronary artery angiography at the same time. There was 1 case of missed diagnosis in coronary CTA. 276 branches, 112 normal blood vessels, coronary angiography showed 275 vascular lesions, 113 normal blood vessels, the results of the two tests were highly consistent. Evaluation of the application value of the two examination methods found that the sensitivity, specificity and accuracy of coronary angiography were 97.94%, 97.94% and 98.97%, respectively, while the coronary CTA were 98.97%, 96.91% and 97.94% respectively. Statistics show no meaning. P> 0.05. Conclusions Coronary angiography and coronary CTA are of high value in the diagnosis of coronary heart disease. However, in contrast, coronary CTA is more safe and reliable. It has less trauma and lower risk to patients and is easily used by clinicians accept.
