目的探讨保留胃迷走神经远端胃癌根治术对原发性胃癌患者术后营养状态的影响。方法选取胃癌患者74例,按随机数字表法分为两组,每组37例。对照组实施传统胃癌根治术治疗,观察组实施保留胃迷走神经远端胃癌根治术,对比两组胃肠激素相关指标、胃肠功能恢复及营养状况。结果术后7 d,观察组胰高血糖素、胃泌素水平明显低于对照组,差异有统计学意义(P<0.05);观察组肛门首次排气时间明显短于对照组,餐后上腹饱胀感及食欲下降发生率明显低于对照组,差异有统计学意义(P<0.05);术后6个月,观察组体质量指数、前白蛋白及白蛋白明显高于对照组,差异有统计学意义(P<0.05)。结论给予原发性胃癌患者保留胃迷走神经远端胃癌根治术可提高治疗效果,促进患者胃肠功能恢复,改善胃肠激素水平,且术后营养状况较好。
Objective To investigate the effect of preserving the gastric vagus nerve distal gastric cancer radical operation on the nutritional status of patients with primary gastric cancer. Methods Totally 74 patients with gastric cancer were divided into two groups according to random number table method, with 37 cases in each group. In the control group, radical gastrectomy was performed in the control group. The observation group was performed radical gastrectomy for distal gastric gastrectomy, and the gastrointestinal hormone related indexes, gastrointestinal function recovery and nutritional status were compared between the two groups. Results The levels of glucagon and gastrin in the observation group were significantly lower than those in the control group on the 7th day after operation (P <0.05). The time of the first evacuation of the anus in the observation group was significantly shorter than that of the control group Abdominal fullness and loss of appetite was significantly lower than the control group, the difference was statistically significant (P <0.05); 6 months after operation, the observation group body mass index, prealbumin and albumin was significantly higher than the control group, The difference was statistically significant (P <0.05). Conclusions Radical gastric conservative treatment of gastric vagal nerve in patients with primary gastric cancer can improve the therapeutic effect, promote the recovery of gastrointestinal function, and improve the level of gastrointestinal hormone in patients with primary gastric cancer, and the nutritional status after operation is good.
