目的探讨子痫前期患者胎盘组织α2-HS-糖蛋白(AHSG)基因mRNA及蛋白表达与胰岛素抵抗的关系。方法检测36例正常足月妊娠妇女(A组)和24例轻度子痫前期患者(B组)及17例重度子痫前期患者(C组)剖宫产前空腹血及新生儿脐血AHSG、血糖、胰岛素水平,采用RT-PCR、Western blot法测定胎盘组织中AHSG基因mRNA及蛋白表达水平。结果B、C组新生儿脐血AHSG水平均显著高于A组(P<0.01)。C组新生儿脐血胰岛素、血糖水平显著低于A组(P<0.05)。A、B、C三组胎盘组织中AHSG基因mRNA表达水平分别为0.57±0.17、0.64±0.14、1.32±0.38,C组显著高于B、A组(P<0.05)。A、B、C三组胎盘组织中AHSG蛋白表达水平分别为0.28±0.11、0.30±0.10、0.58±0.10,C组显著高于A组和B组(P<0.05)。结论子痫前期患者新生儿脐血AHSG水平显著高于正常足月妊娠组新生儿。重度子痫前期患者胎盘组织中AHSG基因mRNA及蛋白表达水平显著增高,胰岛素抵抗在子痫前期发病中有重要作用。
Objective To investigate the relationship between the mRNA and protein expression of α2-HS-glycoprotein (AHSG) in placenta and insulin resistance in preeclampsia. Methods 36 cases of normal term pregnant women (group A) and 24 cases of mild preeclampsia (group B) and 17 cases of severe preeclampsia (group C) were tested for fasting blood before cesarean section and neonatal cord blood AHSG , Blood glucose and insulin levels. The mRNA and protein expression of AHSG in placenta were determined by RT-PCR and Western blot. Results The levels of AHSG in cord blood of neonates in groups B and C were significantly higher than those in group A (P <0.01). The cord blood insulin and blood glucose levels of neonates in group C were significantly lower than those in group A (P <0.05). The mRNA expression levels of AHSG in group A, B and C were 0.57 ± 0.17,0.64 ± 0.14 and 1.32 ± 0.38, respectively, which were significantly higher in group C than those in group B and group A (P <0.05). The levels of AHSG in A, B and C groups were 0.28 ± 0.11, 0.30 ± 0.10 and 0.58 ± 0.10, respectively, which were significantly higher in group C than those in group A and B (P <0.05). Conclusion The level of AHSG in umbilical cord blood of preeclampsia patients is significantly higher than that of normal term pregnancy group. The levels of AHSG mRNA and protein in placenta of patients with severe preeclampsia are significantly increased, and insulin resistance plays an important role in the pathogenesis of preeclampsia.
