目的应用三维步态分析系统,定量分析偏瘫患者下肢的三维运动学特征,确定客观反映脑卒中后下肢偏瘫步态运动学特征的量化指标。方法对27名正常人与39例脑卒中患者采用三维步态分析系统进行运动学参数检测与分析。结果实验组脑卒中患者偏瘫侧髋、膝运动活动范围(ROM);髋伸展和踝背伸角度;髋伸展和踝背伸角度 W/S 比率与正常人群比较差异均有统计学意义(P<0.01)。脑卒中患者偏瘫侧膝 ROM、髋伸展和踝背伸与健侧比较差异有统计学意义(P<0.01);其患侧膝屈曲与健侧比较差异也有统计学意义(P<0.05)。结论脑卒中偏瘫步态下肢的运动学特征:偏瘫侧下肢关节 ROM 受限;患/健侧下肢运动不对称;说明偏瘫下肢的三维运动学参数是描述偏瘫模式的重要指标。
Objective To quantitatively analyze the three-dimensional kinematics characteristics of the lower extremities in patients with hemiplegia by using the three-dimensional gait analysis system, and to determine the quantitative indicators that objectively reflect the kinematic characteristics of the lower limb hemiplegia gait after stroke. Methods 27 normal subjects and 39 stroke patients were detected and analyzed by three-dimensional gait analysis system. Results The hemiplegic lateral hip and knee range of motion (ROM), hip extension and ankle dorsiflexion angle in the experimental group were significantly different from those in the normal group (P < 0.01). There were significant differences in hemiplegia ROM, hip extension and ankle dorsiflexion and contralateral leg between stroke patients and stroke patients (P <0.01). There was also significant difference between flexion and contralateral knee in stroke patients (P <0.05). Conclusions The kinematic characteristics of lower extremity in stroke hemiplegia gait are limited: ROM of hemiplegic lower extremity joint is limited; asymmetric movement of lower extremity in diseased / contralateral hemisphere; the three-dimensional kinematic parameters of lower extremity in hemiplegic hemiplegia are an important index for describing hemiplegia pattern.
