已证实轻链病(Light Chain Disease,简称LCD)是骨髓瘤的变种之一,由浆细胞恶性增生所致。其特征为血和尿中出现大量同一类型的游离轻链,血清中正常免疫球蛋白量在正常范围或减少。临床上有多发性骨髓瘤的表现,更严重的是常有浆细胞白血病的表现。轻链病约占全部多发性骨髓瘤的10％左右。其中多数为单克隆的κ(Kappa)型,少数为单克隆的λ(Lambda)型。双单克隆轻链病亦即κ型和λ型同时存在于患者血和尿中的“双轻链病”却甚罕见。迄今国外报道过二例,国内报道一例。我院也发现一例,现将诊断此病的主要实验室检查报道如下。 病历简要 患者系70岁男性职员。三个月前出现头昏,全身无力,行走困难,易疲劳等症状。血象检查:血色 It has been confirmed that Light Chain Disease (LCD) is one of the variants of myeloma caused by the malignant hyperplasia of plasma cells. It is characterized by the presence of a large number of free light chains of the same type in blood and urine with normal or diminished normal immunoglobulin levels in the serum.
