已证实轻链病(Light Chain Disease,简称LCD)是骨髓瘤的变种之一,由浆细胞恶性增生所致。其特征为血和尿中出现大量同一类型的游离轻链,血清中正常免疫球蛋白量在正常范围或减少。临床上有多发性骨髓瘤的表现,更严重的是常有浆细胞白血病的表现。轻链病约占全部多发性骨髓瘤的10%左右。其中多数为单克隆的κ(Kappa)型,少数为单克隆的λ(Lambda)型。双单克隆轻链病亦即κ型和λ型同时存在于患者血和尿中的“双轻链病”却甚罕见。迄今国外报道过二例,国内报道一例。我院也发现一例,现将诊断此病的主要实验室检查报道如下。 病历简要 患者系70岁男性职员。三个月前出现头昏,全身无力,行走困难,易疲劳等症状。血象检查:血色
It has been confirmed that Light Chain Disease (LCD) is one of the variants of myeloma caused by the malignant hyperplasia of plasma cells. It is characterized by the presence of a large number of free light chains of the same type in blood and urine with normal or diminished normal immunoglobulin levels in the serum. Clinical manifestations of multiple myeloma, more serious is often the performance of plasma cell leukemia. Light chain disease accounts for about 10% of all multiple myeloma. Most of them are kappa type of monoclonal and a few are lambda type of monoclonal. Dual Monoclonal Light-chain Disease, aka “double light chain disease” that is both κ and λ co-existing in the blood and urine of patients, is rare. To date, two cases have been reported abroad and one case reported in China. Our hospital also found a case, now the main laboratory test to diagnose the disease reported below.
