目的 :探讨磁共振成像(MRI)联合动态对比增强磁共振(DCE-MRI)诊断前列腺癌(PCa)的临床价值。资料和方法回顾性分析广西医科大学第一附属医院资料,经穿刺活检及手术取得病理的42例前列腺癌(PCa)及26例前列腺增生(BPH)患者,行MRI平扫,动态对比增强磁共振扫描后采用4D-Tissu软件自动分析并得出时间-信号强度曲线(SI-T曲线),分析MRI信号特征及SI-T曲线类型。比较MRI联合DCE-MRI与PSA在鉴别PCa和BPH的临床价值结果 MRI联合DCE-MRI对诊断前列腺癌具有较高价值,联合应用与PSA方法其敏感度、特异度及准确度分别为:95.2%,84.6%,91.7%;83.3%,53.8%,72.1%。多参数MRI比PSA检查手段的诊断准确度明显提高,差别有统计学意义(χ2=8.273,P=0.041)结论 MRI联合DCE-MRI可较大提高诊断PCa的敏感度、特异度和准确度。
Objective: To investigate the clinical value of magnetic resonance imaging (MRI) combined with dynamic contrast-enhanced magnetic resonance (DCE-MRI) in the diagnosis of prostate cancer (PCa). Materials and Methods The data of 42 cases of prostate cancer (PCa) and 26 cases of benign prostatic hyperplasia (BPH) were obtained from the First Affiliated Hospital of Guangxi Medical University. The data of MRI scan, dynamic contrast enhanced MRI After scanning, the time-signal intensity curve (SI-T curve) was automatically analyzed by 4D-Tissu software to analyze MRI signal characteristics and SI-T curve types. MRI and DCE-MRI in the differential diagnosis of PCa and BPH Clinical value of MRI combined with DCE-MRI in the diagnosis of prostate cancer have high value, combined application and PSA sensitivity, specificity and accuracy were: 95.2% , 84.6%, 91.7%; 83.3%, 53.8%, 72.1%. The diagnostic accuracy of multi-parameter MRI was significantly higher than that of PSA, the difference was statistically significant (χ2 = 8.273, P = 0.041) Conclusion MRI combined with DCE-MRI can greatly improve the sensitivity, specificity and accuracy of the diagnosis of PCa.
