目的探讨提高高位空肠平滑肌肿瘤的诊治水平 .方法回顾性分析 12例距十二指肠悬韧带 15 cm以内空肠平滑肌肿瘤病例 .结果本组 12例中 ,距十二指肠悬韧带小于 5 cm 4例 , 5～ 10 cm 6例 ,大于 10 cm 2例 ,均为腔外型 .术前误诊率为 75 0% ,手术切除率 100% ,其中平滑肌肉瘤 4例 ,平滑肌瘤 8例 .生存 9例 ,死亡 3例 ,生存最长者达 20年 . 5年生存率为 58 3% .结论本病误诊率较高 ,提高诊断水平至关重要 ,出血危及生命者应行剖腹探查术 ,根治性切除预后较好 ,对复发者行再次手术切除肿瘤可提高生存率 .
Objective To investigate the diagnosis and treatment of high jejunum smooth muscle tumors.Methods 12 cases of jejunum smooth muscle tumors within 15 cm from the duodenal ligament were retrospectively analyzed.Results In 12 cases of this group, the duodenal ligament was less than 5 cm 4 Cases, 5 ~ 10 cm 6 cases, more than 10 cm 2 cases were cavity appearance. Preoperative misdiagnosis rate was 75.0%, surgical resection rate was 100%, of which 4 cases of leiomyosarcoma, 8 cases of leiomyoma. 9 cases died in 3 cases, the longest survival of 20 years .5-year survival rate of 58 3% .Conclusion The misdiagnosis rate of the disease is high, to improve the diagnostic level is essential, life-threatening bleeding should be performed laparotomy, Radical resection of the prognosis is good, the recurrence of surgical resection of the tumor can increase the survival rate.
