以膀胱叩击诱发自主神经过反射(AHR)法对15例胸腰段脊髓损伤截瘫病人、15例颈髓损伤四肢瘫病人进行检查。膀胱叩击前和叩击后分别测定血液中主要引起血压升高的血管活性物质去甲肾上腺素(NE)、肾上腺素(E)、肾素(R)和血管紧张素Ⅱ(AⅡ)。结果表明,叩击膀胱可引起四肢瘫病人发生AHR,同时血中NE和R明显升高。通过膀胱叩击时四肢瘫和截瘫病人血管活性物质的变化,可认为:(1)由于NE为交感神经末梢传导神经冲动的主要神经介质,AHR时血浆NE浓度增高,表明AHR是由于交感神经兴奋所致。(2)AHR时,由于仅血清R增高而血浆E和血清AⅡ无明显改变,故其血压变化与肾上腺的功能变化和肾素-血管紧张素系统的改变无关。
15 cases of thoracolumbar spinal cord injury paraplegia patients and 15 cases of cervical spinal cord injury quadriplegia patients were examined by bladder percussion-induced autonomic nerve reflex (AHR). Before and after percussion, the vasoactive substances norepinephrine (NE), epinephrine (E), renin (R) and angiotensin II (AII), which are mainly caused by the increase of blood pressure, were measured in blood. The results showed that knocking the bladder can cause AHR in patients with quadriplegia, meanwhile, NE and R in blood are obviously increased. Through the bladder tapping quadriplegia and paraplegia patients vasoactive substances changes, can be considered: (1) because NE is the main nerve mediators of sympathetic nerve conduction impulse nerve terminals, AHR plasma NE concentration increased, indicating that AHR is due to sympathetic excitement Due. (2) At the time of AHR, only changes of serum R and changes of plasma E and serum A II showed no significant changes. Therefore, the changes of blood pressure were not related to adrenal function changes and renin-angiotensin system changes.
