目的:以犬为研究对象,研究不同温度(45℃,50℃,55℃,60℃,65℃)的热灌注液经肝动脉灌注对犬肝内温度、肝组织学及肝、肾功能的动态影响,为介入性热化疗的临床应用提供实验依据。方法:经犬肝动脉分别灌注入45℃、50℃、55℃、60℃、65℃生理盐水180 ml,持续30 min。观察并比较热灌注前后肝脏穿刺活检及肝、肾功能的动态变化情况。结果:1.45℃、50℃、55℃、60℃、65℃肝动脉热灌注时,肝内中心测得的温度分别为(37.9±0.5)℃、(38.6±0.3)℃、(40.5±0.3)℃、(42.1±0.5)℃和(45.8±0.6)℃,谷丙转氨酶(alanine aminotransferase,ALT)、谷草转氨酶(aspartate transaminase,AST)于60℃灌注后24 h开始升高,72 h达到高峰(P< 0.01),168 h后逐渐下降,216 h后恢复正常;总胆红素(total bilirubin,T-BIL)、白蛋白(albumin,ALB)、γ-谷氨酰基转移酶(γ-glutamyltransferase,γ-GT)无明显变化(P< 0.05)。2.肝组织可见一过性淋巴细胞浸润、肝细胞混浊肿胀,168h后恢复正常。3.尿素氮(blood urea nitrogen,BUN)、肌酐(creatinine,Cr)无显著性变化(P< 0.05),尿酸(uric acid,UA)呈一过性增高(P< 0.05)。4.45~55℃热灌注对肝肾功能无显著影响,65℃热灌注后肝肾功能恢
OBJECTIVE: To study the effects of hepatic artery perfusion of thermal perfusate at different temperatures (45 ℃, 50 ℃, 55 ℃, 60 ℃, 65 ℃) on the intrahepatic liver temperature, liver histology and liver and kidney function Dynamic impact, provide experimental evidence for the clinical application of interventional thermochemotherapy. Methods: 180 ml normal saline at 45 ℃, 50 ℃, 55 ℃, 60 ℃ and 65 ℃ were injected into canine hepatic artery respectively for 30 min. The dynamic changes of liver biopsy and liver and kidney function before and after thermal perfusion were observed and compared. Results: The intrahepatic center measured temperatures were (37.9 ± 0.5) ℃, (38.6 ± 0.3) ℃ and (40.5 ± 0.3) ° C at 1.45 ℃, 50 ℃, 55 ℃, 60 ℃ and 65 ℃ respectively. (42.1 ± 0.5) ℃ and (45.8 ± 0.6) ℃ respectively. Alanine aminotransferase (ALT) and aspartate transaminase (AST) began to increase at 24 h after perfusion at 60 ℃ and peaked at 72 h P <0.01), and then decreased gradually after 168 h and returned to normal after 216 h. Total bilirubin (T-BIL), albumin (ALB), γ-glutamyltransferase γ-GT) did not change significantly (P <0.05). 2. Hepatic tissue showed a transient lymphocytic infiltration, swelling of turbid cells, 168h after the return to normal. Blood urea nitrogen (BUN), creatinine (Cr) showed no significant change (P <0.05), uric acid (UA) showed a transient increase (P <0.05). 4.45 ~ 55 ℃ thermal perfusion on liver and kidney function had no significant effect, after 65 ℃ hot perfusion liver and kidney function recovery
