目的:对200例宫腔镜患者术前应用盐酸戊乙奎醚,探讨其安全性和效果评价。方法:随机抽取200例行宫腔镜手术患者,ASAⅠ~Ⅱ级。随机分为两组,对照组:100例,阿托品0.5mg;治疗组:100例,盐酸戊乙奎醚组0.5mg。两组均于术前5min给药,均给予复合丙泊酚麻醉,首次剂量1.5mg·kg~(-1)。患者发生扭动时再追加首剂的一半。术中监测HR、SPO_2、MAP变化,丙泊酚用量及出血量,观察患者清醒后达到OAA/S 2分,5分时所需时间,镇痛、镇静效果及并发症。结果:两组患者术后HR,MAP较术前均有明显降低(P<0.05);但术后5min时治疗组HR、MAP下降较对照组显著(P<0.05);两组间SPO_2差异无统计学意义(P>0.05);丙泊酚使用总量两组间P>0.05,但单位时间内治疗组丙泊酚使用量与对照组比较P<0.05。对照组患者术中出血量较治疗组P<0.05。口干指标两组均较术前大(P<0.05),但两组间P>0.05;镇痛评分及记忆程度治疗组较对照组效果良好(P<0.05)。结论:盐酸戊乙奎醚复合丙泊酚更适合用于宫腔镜手术麻醉。
Objective: To investigate the safety and efficacy of penehyclidine hydrochloride in preoperative application of 200 cases of hysteroscopy. Methods: 200 cases underwent hysteroscopic surgery, ASA Ⅰ ~ Ⅱ grade. Randomly divided into two groups, the control group: 100 cases, atropine 0.5mg; treatment group: 100 cases, penehyclidine hydrochloride group 0.5mg. Both groups were given 5 min before operation, and were given propofol anesthesia for the first time. The first dose was 1.5 mg · kg ~ (-1). Half of the first dose is added when the patient twists. Intraoperative monitoring of HR, SPO_2, MAP changes, the amount of propofol and bleeding, observed in patients with awake to OAA / S 2 points, 5 minutes time required, analgesic, sedation and complications. Results: HR and MAP in both groups were significantly lower than those before surgery (P <0.05). However, HR and MAP in treatment group were significantly lower than those in control group at 5 minutes after operation (P <0.05). There was no significant difference in SPO_2 between the two groups Statistical significance (P> 0.05). The total amount of propofol use between the two groups P> 0.05, but the use of propofol per unit time in the treatment group compared with the control group P <0.05. The control group patients with intraoperative bleeding than the treatment group P <0.05. (P <0.05), but P> 0.05 between the two groups; analgesic score and memory degree of the treatment group were better than the control group (P <0.05). Conclusion: Penehyclidine hydrochloride combined with propofol is more suitable for hysteroscopic surgery.
