目的探讨透明帽辅助肠镜插入法在肠镜教学中的作用。方法使用富士4450型肠镜(长度1.2m),由1名有经验的肠镜操作者对该院4名初学肠镜操作的医生(操作例数为150~300例)进行透明帽辅助肠镜操作法培训,培训后选取2015年12月—2016年5月肠镜检查的800例患者,按照内镜检查登记先后顺序编号,采用完全随机化方法将所有患者均分为二组,透明帽组与非透明帽组各400例。分析二种检查方法在操作成功率、进镜时间、息肉检出以及检查过程中患者疼痛程度评分的差异。结果透明帽组的平均进镜时间为(11.13±5.66)min,明显短于常规组的(11.58±5.71)min,差异具有统计学意义(P<0.05);透明帽组息肉总检出率为52.75%,显著高于常规组的37.25%,差异具有统计学意义(P<0.001);≤5mm息肉的检出率为44.25%,显著高于常规组的31.25%,差异具有统计学意义(P<0.001);透明帽组平均息肉检出个数为1.19±1.45,明显多于常规组的0.88±1.40,差异具有统计学意义(P<0.001);进镜操作成功率及疼痛评分方面二组差异无统计学意义。结论透明帽辅助肠镜插入法可使肠镜初学者缩短进镜时间并提高息肉检出率。
Objective To explore the role of transparent cap-assisted colonoscopy in colonoscopy teaching. The average number of polyp detected in the transparent cap group was 1.19 ± 1.45, which was significantly higher than that of the conventional group (0.88 ± 1.40, P <0.001).
