目的研究乌司他丁(UTI)在婴幼儿体外循环(CPB)手术中对氧自由基的影响。方法 36例拟在CPB下行心内直视手术的先天性心脏病婴幼儿,随机分为两组,每组18例。乌司他丁组(UTI组)转流前经中心静脉泵入乌司他丁5000 U/kg,10 min内完成;对照组(C组)给予等量平衡液。分别于转流前和转流后从中心静脉采血检测血浆超氧化物歧化酶(SOD)、丙二醛(MDA)水平。结果 1与转流前相比,转流后C组MDA含量明显升高,而SOD明显减少(P<0.05);2组间比较,转流前两组的MDA和SOD含量变化不明显,转流后C组MDA含量明显高于UTI组,而SOD含量显著低于UTI组(P<0.05)。结论乌司他丁可抑制婴幼儿CPB手术中氧自由基的产生,从而减轻心肌缺血再灌注损伤。
Objective To investigate the effect of ulinastatin (UTI) on oxygen free radicals during cardiopulmonary bypass (CPB) in infants and young children. Methods Thirty-six cases of congenital heart disease undergoing open-heart surgery in CPB were randomly divided into two groups, 18 cases in each group. The ulinastatin group (UTI group) was injected with ulinastatin (5000 U / kg) through the central venous pump within 10 minutes before the infusion. The control group (C group) was given equal amount of balancing fluid. Plasma levels of superoxide dismutase (SOD) and malondialdehyde (MDA) were measured before and after bypass from the central venous blood respectively. Results 1 Compared with that before the commutation, the content of MDA in C group increased significantly and the SOD decreased significantly (P <0.05). The contents of MDA and SOD in the two groups before commutation did not change obviously The content of MDA in group C was significantly higher than that in group UTI, while the content of SOD in group C was significantly lower than that in group UTI (P <0.05). Conclusion Ulinastatin can inhibit the production of oxygen free radicals in infants and young children undergoing CPB surgery and thus reduce myocardial ischemia-reperfusion injury.
