我院自1986～1990年共收治22例结核性脑膜炎(结脑),选用含SHZ并氟美松方案,取得较为满意的效果。病人年龄1～56岁。脑膜炎型11例(50％),伴颅神经损害或颅内定位体征的脑膜脑炎型9例(40.9％),有脊髓损害的蛛网膜下腔梗阻的脑脊髓膜炎型2例;中晚期和迁延期占77.7％。合并肺结核20例,其中原发型3例、血播型16例、浸润型1例。治疗方案 SM0.75日1次肌注、INH0.3日顿服并静脉滴注0.6～0.9、PZA0.5日3次口服。儿童用量酌减;全部并用氟美松静脉滴注0.1～0.2mg/kg;3例曾椎管给药氟美松,10例曾加用RFP。文献报道,多采用去氢或氢化可的松,并主张1个月后开始减量,疗程不超过3个月。本组应用氟美松是在脑脊 Twenty-two patients with tuberculous meningitis (tuberculous meningitis) were treated in our hospital from 1986 to 1990, and SHZ and dexamethasone-containing regimens were selected to achieve satisfactory results.
