目的比较IDEAL序列和FS-FSE序列对减轻脊柱金属内固定相关伪影面积大小、脂肪抑制均匀性、椎管内结构的显示能力。方法选取脊柱金属内固定术后行脊柱MRI检查的患者40例,所有受检者脊柱内固定均为含量89%的钛六铝四钒(美敦力枢法模)。分别选取矢状位和轴位IDEAL-T_2WI、FS-FSE-T_2WI正中层面图像对金属伪影面积进行测量分析。脂肪抑制效果评价和椎管内结构显示效果评价均采用3分法。图像噪声评价测量脊髓ROI和背景ROI区域的信号强度与标准差,计算信噪比和对比度噪声比。结果累及3个腰椎椎弓板总的金属伪影面积在矢状位和轴位IDEAL-T_2WI和FS-FSE-T_2WI上分别为(2168±1454)mm~2、(4278±1259)mm2(P<0.003)和(1327±321)mm~2、(2777±1185)mm~2(P<0.001);IDEAL-T_2WI和FS-FSE-T_2WI的脂肪抑制评分观察者1和观察者2分别为1.83±0.39、0.60±0.81(P<0.05)和1.90±0.30、0.48±0.60(P<0.05);IDEAL-T_2WI和FSFSE-T_2WI的椎管内结构显示评分观察者1和观察者2分别为1.25±0.78、0.65±0.89(P<0.05)和1.40±0.67、0.60±0.78(P<0.05);图像噪声SNR和CNR在IDEAL-T_2WI和FS-FSE-T_2WI分别为97.13±33.55、42.11±12.53(P<0.05)和38.52±21.05、46.52±17.44(P>0.05)。结论相较于FS-FSE,IDEAL能显著减轻脊柱内固定金属相关伪影,提供更加均匀的脂肪抑制图像,提高椎管内和周围软组织结构的显示效果。
Objective To compare the performance of IDEAL and FS-FSE sequences in reducing the size of the artifact-related area of spinal metal internal fixation, the uniformity of fat suppression, and the structure of the spinal canal. Methods Forty patients with spine MRI after spine metal internal fixation were selected. All patients underwent spinal internal fixation with Titanium-hexa-tetra-vanadium (Medtronic model) with 89% content. The sagittal and axial IDEAL-T_2WI and FS-FSE-T_2WI median images were selected to measure the metal artifact area respectively. Evaluation of fat suppression and spinal structure showed that the results were evaluated using 3 points. Image Noise Assessment Signal strength and standard deviation of the spinal ROI and background ROI region were measured and the ratio of signal to noise and contrast noise was calculated. Results The total metal artifacts involved in the three lumbar discs were (2168 ± 1454) mm2, (4278 ± 1259) mm2 (P <0.003) and (1327 ± 321) mm ~ 2, (2777 ± 1185) mm ~ 2, respectively (P <0.001). The fat suppression scores of IDEAL-T 2 WI and FS-FSE-T 2 WI were 1.83 ± 0.39,0.60 ± 0.81 (P <0.05) and 1.90 ± 0.30,0.48 ± 0.60 (P <0.05). The spinal canal structures of IDEAL-T_2WI and FSFSE-T_2WI showed that the scores of observer 1 and observer 2 were 1.25 ± The SNR and CNR of image noise were 97.13 ± 33.55 and 42.11 ± 12.53 (P <0.05) in IDEAL-T 2WI and FS-FSE-T 2WI groups, P <0.05, 0.78,0.65 ± 0.89 and 1.40 ± 0.67,0.60 ± 0.78 <0.05) and 38.52 ± 21.05, 46.52 ± 17.44 (P> 0.05). Conclusion Compared with FS-FSE, IDEAL can significantly reduce the fixation-related artifacts in the spine, provide a more uniform fat-suppressed image, and improve the visualization of soft tissue structures in and around the spinal canal.
