目的:探讨缺氧诱导因子-1α(HIF-1α)和葡萄糖转运蛋白-1(GLUT-1)在子宫内膜癌中的表达及其与预后的关系。方法:选取2010年7月~2013年8月在该院行手术切除的子宫内膜腺癌存档蜡块标本74例,另选取同期进行手术切除或门诊刮取的子宫内膜不典型增生病例23例、复杂性增生病例18例及正常子宫内膜15例。采用免疫组化SP二步法检测不同组织中HIF-1α和GLUT-1的表达情况,分析其与临床病理特征之间的关系。结果:HIF-1α在子宫内膜癌、子宫内膜不典型增生、复杂性增生和正常子宫内膜中的阳性表达率分别为79.7%、52.2%、25.0%和6.7%,差异均有统计学意义(P<0.05);GLUT-1在子宫内膜癌、子宫内膜不典型增生、复杂性增生和正常子宫内膜中的阳性表达率分别为75.7%、43.5%、11.1%和0.0%,差异均有统计学意义(P<0.05);HIF-1α和GLUT-1表达与病理分期、组织学分级、浸润深度及淋巴结转移有关(P<0.05),病理分期越高、组织分级越低、浸润深度越深、有淋巴结转移,HIF-1α和GLUT-1的阳性表达率越高;HIF-1α与GLUT-1表达呈正相关(r=0.654,P<0.05)。结论:HIF-1α和GLUT-1在子宫内膜癌病理进程中发挥重要作用,可以作为判定子宫内膜癌恶性程度及预后的敏感指标。
Objective: To investigate the expression of hypoxia-inducible factor-1α (HIF-1α) and glucose transporter-1 (GLUT-1) in endometrial carcinoma and its relationship with prognosis. Methods: Totally 74 cases of endometrial adenocarcinoma were collected from July 2010 to August 2013 in our hospital. Another case of endometrial atypical hyperplasia was selected by surgical resection or outpatient operation Cases, 18 cases of complicated hyperplasia and 15 cases of normal endometrium. The expression of HIF-1α and GLUT-1 in different tissues was detected by immunohistochemical SP two-step method and its relationship with clinicopathological features was analyzed. Results: The positive rates of HIF-1α in endometrial carcinoma, endometrial dysplasia, complex hyperplasia and normal endometrium were 79.7%, 52.2%, 25.0% and 6.7%, respectively, with statistical differences (P <0.05). The positive expression rates of GLUT-1 in endometrial carcinoma, endometrial dysplasia, complex hyperplasia and normal endometrium were 75.7%, 43.5%, 11.1% and 0.0% (P <0.05). The expressions of HIF-1α and GLUT-1 were correlated with the histological grade, histological grade, depth of invasion and lymph node metastasis (P <0.05) The deeper the depth of invasion, lymph node metastasis, HIF-1α and GLUT-1 positive expression rate higher; HIF-1α and GLUT-1 expression was positively correlated (r = 0.654, P <0.05). Conclusions: HIF-1α and GLUT-1 play an important role in the pathological process of endometrial carcinoma, which can be used as a sensitive index to determine the malignancy and prognosis of endometrial carcinoma.
