目的了解成都市乡镇卫生院更换数字化医用X射线摄影系统(digital radiology,DR)前后放射工作人员个人放射剂量水平,探讨更换DR并改造机房防护对工作人员的防护效果。方法 2011-2014年对618人更换DR前后分别有2年时间的放射工作从业人员进入监测队列。采用热释光剂量测量方法对放射工作人员进行监测,用SPSS 18.0进行数据分析。结果 2012年和2013年分别有145家和271家乡镇卫生院更换DR并对机房进行防护改造,涉及工作人员618名,更换前放射工作人员的年人均剂量的中位数为0.485 m Sv/a,年集体剂量为1.19人·Sv,年有效剂量小于5 m Sv/a的人员有598人(96.76%),有17人(2.75%)的剂量在5 m Sv/a到20 m Sv/a,有2人(0.32%)的剂量超过20 m Sv/a,无人员超过年50 m Sv/a国家标准规定放射工作人员年剂量限值。更换后放射工作人员的年人均剂量的中位数为0.430 m Sv/a,年集体剂量为1.02人·Sv,年有效剂量小于5 m Sv/a的人员有603人(97.57%),有9人(1.46%)的剂量在5 m Sv/a到20 m Sv/a,有5人(0.81%)的剂量超过20 m Sv/a,无人员超过年50 m Sv/a国家标准规定放射工作人员年剂量限值。结论成都市乡镇卫生院更换DR并改造机房防护后放射工作人员个人放射剂量水平有所降低。
Objective To understand the radiation dose level of radiation workers before and after the digital medical radiology system (DR) was replaced by the township hospitals in Chengdu, and to explore the protective effect of replacing the DR and rehabilitating the staff in hospital room protection. Methods From 2011 to 2014, the radiological practitioners who had been active for two years before and after the change of DR in 618 people entered the monitoring cohort. Radiation workers were monitored by thermoluminescence dose measurement method and analyzed by SPSS 18.0. Results In 2012 and 2013 respectively, 145 and 271 township hospitals changed their DRs and renovated the engine room, involving 618 staff members.
