目的探讨嘉兴市秀洲区剖宫产的各种影响因素,制定干预措施,使剖宫产率在合理范围,从而减少产科并发症,保障母婴健康。方法对嘉兴市秀洲区妇保院2007-2011年的剖宫产率,剖宫产指征变化进行回顾性分析。结果通过单因素分析和多因素Logistic回归,得出产妇自身愿意剖宫产、医生建议剖宫产、周围产妇采用剖宫产、新生儿体重、年龄和孕期增重对采用剖宫产具有正向影响,而产次、医生建议自然分娩和产妇自身愿意自然分娩对降低剖宫产具有作用。结论剖宫产率下降停滞,是多方面原因造成的,应加强产妇宣教和孕产妇保健,提高疤痕子宫的阴道分娩率,减少因社会因素而行剖宫产的产妇数量。
Objective To explore various influencing factors of cesarean section in Xiuzhou District of Jiaxing City and to make interventions to make cesarean section rate within a reasonable range so as to reduce the obstetric complications and ensure the health of mother and infant. Methods A retrospective analysis was made on the changes of cesarean section rate and cesarean section indications from 2007 to 2011 in Xiuzhou District, Jiaxing. Results By univariate analysis and multivariate Logistic regression, we found that the maternal womens willingness to cesarean section, the doctor recommended caesarean section, the surrounding maternal adoption of cesarean section, weight of newborns, age and weight gain during pregnancy had positive Influence, and parity, the doctor suggested that natural childbirth and mothers themselves willing to natural childbirth on reducing cesarean section has a role. Conclusions The decline of cesarean section rate is caused by many reasons. It is necessary to strengthen maternal mission education and maternal health care, improve vaginal delivery rate in scar-shaped uterus, and reduce the number of mothers who have cesarean section due to social factors.
