[目的]探讨氩氦刀冷冻、射频消融(RFA)及微波凝固(MCT)治疗兔VX2肝癌的疗效。[方法]取制作成功VX2肝癌模型的新西兰大白兔45只,随机分为5组,每组9只,即氩氦刀冷冻组(A组)、射频消融组(B组)、微波凝固组(C组)、手术切除组(D组)和对照组(E组)。观察肿瘤残留和转移情况以及兔生存期。[结果]①在A、B、C、D、E组肝内肿瘤残留分别为2、4、5、0、9只,差异有显著性(χ2=20.700,P=0.000);肝内转移分别为1、3、4、5、9只,差异有显著性(χ2=15.652,P=0.004);腹腔种植转移率分别为2、5、6、0、1只,差异有显著性(χ2=13.894,P=0.008);肺部及腹腔淋巴结转移各组均为9只,差异无显著性。②A、B、C、D组与E组比较平均生存期有显著性差异(F=73.084,P=0.000);A、B、C和D组平均生存期均显著高于E组;A、D组平均生存期显著高于B、C组;A组和D组之间、B组和C组之间平均生存期差异无显著性。[结论]兔VX2肝癌采用氩氦刀冷冻治疗在减少肿瘤残留和转移以及延长生存期方面,均优于RFA和MCT治疗;而RFA治疗和MCT治疗疗效相当。
[Objective] To investigate the curative effect of argon-helium-knife cryosurgery, radiofrequency ablation (RFA) and microwave coagulation (MCT) in the treatment of VX2 liver cancer in rabbits. [Method] Forty-five New Zealand white rabbits were randomly divided into 5 groups (n = 9 each), which were cryoablation group (A group), RFA group (B group), microwave coagulation group C group), resection group (D group) and control group (E group). The residual tumor and metastasis were observed as well as the survival of rabbits. [Results] ①There were 2, 4, 5, 0, and 9 intrahepatic tumor residues in groups A, B, C, D and E respectively (χ2 = 20.700, P = 0.000) (Χ2 = 15.652, P = 0.004). The rate of peritoneal implantation metastasis was 2, 5, 6, 0 and 1, respectively, and the difference was significant (χ2 = 13.894, P = 0.008). There were 9 in each group of lung and abdominal lymph node metastasis, the difference was not significant. The average survival time of group A, B, C and D was significantly different from that of group E (F = 73.084, P = 0.000); the average survival time of group A, B, C and D were significantly higher than that of group E; The mean survival time of group A was significantly higher than that of group B and C (P <0.05). There was no significant difference in mean survival between group A and group D, group B and group C [Conclusion] The cryogenic treatment of rabbit VX2 hepatocarcinoma with cryoablation is superior to RFA and MCT in reducing the residual tumor, metastasis and prolonging the survival period of rabbit VX2 hepatocellular carcinoma. However, RFA and MCT are equally effective.
