目的:研究螺旋CT在结肠癌检查中的临床应用价值。方法:对15例经手术病理证实的结肠癌病例行螺旋CT检查,对其2D多平面重建图像(MPR)、3D透明法(Raysum)、表面遮盖法(SSD)、仿真纤维内窥肠镜(CTVE)图像进行回顾性分析。结果:15例结肠癌的CT表现为大小不等的软组织肿块,与肠管关系密切,可挤压并推移周围肠管移位。部分块影内可有囊变灶、低密度坏死区及细小钙化灶。部分肠管壁局限性或广泛性不均匀增厚,增强扫描肿块有明显不均匀强化,中央囊变或坏死区不强化。部分肠腔变窄,其中一例横结肠局部呈环形缩窄,CTVE图像显示缩窄处肠腔菜花状软组织结节突向肠腔内,SSD图像清楚地显示缩窄处的位置及病变肠段的长度。另有一例乙状结肠与降结肠交界处肠腔完全梗阻、闭塞,MPR图像准确地显示病变肠腔部位,手术证实定位准确,且该处肠管壁局部增厚、肿块突向腔内并堵塞肠腔。结论:将多排螺旋CT的轴位图像、MPR及三维SSD、Raysum、CTVE图像有机地结合,可以较准确地进行病变定位、良恶性定性诊断,为临床诊治结肠肿瘤提供直观、可靠的科学依据,具有良好的临床价值和应用前景。
Objective: To study the clinical value of spiral CT in the examination of colon cancer. Methods: Fifteen cases of colon cancer confirmed by surgery and pathology were examined by spiral CT. The images of MPR, Raysum, SSD, CTVE) images were retrospectively analyzed. Results: The CT findings of 15 cases of colon cancer showed soft tissue mass with different sizes, closely related to the intestine and could be squeezed and shifted around the intestine. Part of the shadow can have cystic lesions, low-density necrosis and small calcification. Some limitations of intestinal wall or extensive heterogeneity thickening, enhanced scan mass obvious heterogeneous enhancement, central cystic degeneration or necrosis area is not enhanced. Part of the intestine narrowed, one case of transverse colon was ring-shaped narrowing, CTVE image shows narrow enteral cauliflower soft tissue nodules protruding intestine, SSD image clearly shows the location of narrowing and lesion length of the bowel . Another case of sigmoid colon and descending colon junction complete obstruction, occlusion, MPR images accurately show the lesion in the intestine, the operation confirmed that the positioning accuracy, and where the intestinal wall thickening locally, the mass protruding into the cavity and block the intestine . Conclusion: Combining axial images, MPR, 3D SSD, Raysum and CTVE images of multi-slice spiral CT can accurately locate lesions, diagnose benign and malignant lesions and provide an intuitive and reliable scientific basis for clinical diagnosis and treatment of colon tumors , Has good clinical value and application prospects.
