目的探讨急性脑卒中早期心电图及心肌酶谱和肌钙蛋白临床变化特点。方法对47例急性脑卒中早期心电图及心肌酶谱和肌钙蛋白资料分析。结果47例急性脑卒中早期患者中,20例有心电图异常(42%),出血性卒中的心电图异常率高于缺血性卒中组(P<0.05);心肌酶谱改变16例(34.0%),20例心电图异常的患者中,有心肌酶谱改变的16例(80%),出血性卒中组心肌酶谱水平较缺血性卒中组高(P<0.05);20例心电图异常患者中cTnT阳性者为8例,其中出血性卒中组有5例,缺血性卒中组有3例,两组比较差异无显著意义;丘脑基底节区病灶合并心电图异常明显高于脑叶病灶(P<0.05)。结论急性脑卒中早期心电图及心肌酶谱和肌钙蛋白变化与脑血管疾病类型与部位有关。
Objective To investigate the clinical features of early electrocardiogram, myocardial enzymes and troponin in acute stroke. Methods 47 cases of acute stroke early electrocardiogram and myocardial enzymes and troponin data analysis. Results Of the 47 patients with early acute stroke, 20 had abnormal electrocardiogram (42%) and the abnormal electrocardiogram (ECG) of hemorrhagic stroke was higher than that of ischemic stroke group (P <0.05) , Cardiac abnormalities in 20 patients with ECG changes in 16 cases (80%), myocardial enzymes in hemorrhagic stroke group was higher than ischemic stroke group (P <0.05); cTnT positive in 20 patients with abnormal ECG (N = 8). There were 5 cases in hemorrhagic stroke group and 3 cases in ischemic stroke group. There was no significant difference between the two groups. The abnormality of electrocardiogram in the thalamic basal ganglia area was significantly higher than that in the lobar area (P <0.05) . Conclusion The changes of ECG and myocardial enzymes and troponin in early stroke are related to the type and location of cerebrovascular disease.
