目的 对影响冠心病冠状动脉侧支循环形成的相关因素进行评估。方法 回顾性分析 142例经冠状动脉造影证实管腔狭窄≥ 75 %的冠心病人的侧支循环开放情况 ,以及与高血压、冠状动脉病变部位及左室后壁厚度的关系。结果 在高血压组侧支循环为Ⅱ~Ⅲ级形式的占 70 .3% ,而在非高血压组占44 .35 % ,差异有显著性 ( R =0 .36 2 ,P <0 .0 1) ;右冠状动脉病变的Ⅱ~Ⅲ级形式占 6 1.91% ,而在左冠脉病变中仅占 40 .5 1% ,差异有显著性 ( R =0 .40 0 ,P <0 .0 1) ;侧支循环开放程度与左室后壁厚度呈显著正相关。结论 冠状动脉侧支循环开放数量在高血压和右冠状动脉病变患者中明显增加 ,左心室后壁厚度与侧支循环开放成正相关。
Objective To evaluate the related factors that affect the formation of coronary collateral circulation in patients with coronary heart disease. Methods A retrospective analysis of 142 patients with coronary artery stenosis ≥75% confirmed by coronary angiography was performed to investigate the relationship between collateral circulation and the location of hypertension, coronary lesion and the thickness of posterior wall of left ventricle. Results In the hypertensive group, the collateral circulation accounted for 70.3% in the form of grade Ⅱ ~ Ⅲ, while in the non-hypertension group, 44.35%, the difference was significant (R = 0.362, P <0. 0 1). Grade Ⅱ ~ Ⅲ of right coronary artery lesions accounted for 6 1.91%, while only 40.51% of left coronary artery lesions, the difference was significant (R = 0.40 0, P <0. 0 1). The extent of open collateral circulation was positively correlated with the thickness of left ventricular posterior wall. Conclusions The number of coronary collateral circulation is significantly increased in patients with hypertension and right coronary artery disease. The thickness of left ventricular posterior wall is positively correlated with the opening of collateral circulation.
