患者,女性,8岁,自出生时发现其左足第2、3趾及右足第3、4趾“截指”畸形,左手并指及右手食、中、环指有环形带,并伴有唇裂(3岁时已整形)。随着年龄的增长,右手手指环形带加深,冬季出现肢体发凉及疼痛。查体:左手食中、环、小指并指畸形,中节中部以远缺失,右手食、中、环指近节有环形带,远端肿胀,毛细血管充盈减慢,食环指屈伸正常,中指屈伸略受限(见图1)。左足第2、3趾及右足第3、4趾近侧指间关节以远“截指”畸形(见图2)。X线示双手指骨及足趾骨未见异常。入院后行右手食、中、环指挛缩带分期手术。先行3指掌侧半周切除,3个月后行背侧半周切除,术中见挛缩带深达腱鞘,中指屈肌腱被压迫,切除部分腱鞘,使之屈伸达正常,术中采用Z形皮瓣延长矫形,术后远段肿胀消失,随访半年
Patient, female, 8 years old, found at birth that the second and third digits of the left foot and the fourth and fourth digits of the right foot “cut off” deformity.
