目的评估荧光二极管(LED)直接涂片法、酸性罗氏培养法、Bactec MGIT 960培养法检测结核分枝杆菌临床应用价值。方法 317例临床确诊结核病患者痰样本,每例痰样本均分为3份,分别采用LED直接涂片法、酸性罗氏培养法和Bactec MGIT 960培养法进行检测,比较三种方法培养阳性率,以及阳性率较高的两种检测方法培养所需时间、污染率。结果 317例痰液标本中,Bactec MGIT960培养法、酸性罗氏培养法阳性率均高于LED直接涂片法阳性率,差异具有统计学意义(χ2=97.71、44.69,P<0.01);Bactec MGIT 960培养法阳性率显著高于酸性罗氏培养法,差异具有统计学意义(χ~2=11.80,P<0.01)。酸性罗氏培养法和Bactec MGIT 960培养法的检出时间分别为33.0 d(25.0~40.0 d)、14.0 d(9.0~18.0 d),比较差异有统计学意义(Z=-19.601,P<0.01)。317例痰液标本中,酸性罗氏培养法污染率为4.7%(15/317),Bactec MGIT 960培养法污染率为5.0%(16/317),比较差异无统计学意义(χ2=0.03,P>0.05)。结论 Bactec MGIT 960培养法可以显著提高培养阳性率、缩短培养时间。
Objective To evaluate the clinical application of fluorescent direct diode (LED) smear, acid Roche culture and Bactec MGIT 960 culture to detect Mycobacterium tuberculosis. Methods A total of 317 sputum samples were collected from 317 TB patients. The samples of sputum from each patient were divided into three groups. The samples were examined by LED direct smear method, acidity Roche culture method and Bactec MGIT 960 culture method respectively. The positive rate of culturing was compared with three methods. The positive rate of the two detection methods of culture time, pollution rate. Results The positive rates of Bactec MGIT960 and Acid Roche were higher than those of direct smear LED in 317 sputum samples (χ2 = 97.71, 44.69, P <0.01). Bactec MGIT 960 The positive rate of culture was significantly higher than that of acidic Roche culture, the difference was statistically significant (χ ~ 2 = 11.80, P <0.01). The detection time was 33.0 d (25.0 ~ 40.0 d) and 14.0 d (9.0 ~ 18.0 d), respectively, with significant difference between acidic Roche culture and Bactec MGIT 960 culture (Z = -19.601, P <0.01) . Among 317 sputum samples, the contamination rate of Acid Roche culture was 4.7% (15/317) and that of Bactec MGIT 960 culture method was 5.0% (16/317), the difference was not statistically significant (χ2 = 0.03, P > 0.05). Conclusion Bactec MGIT 960 culture method can significantly improve the positive rate of culture and shorten the culture time.
