目的我们通过对刚出生的不同类型的高危新生儿进行脐血的检测,研究促红细胞生成素、内皮素能否作为围产新生儿危险因素的更为可靠、灵敏的实验室指标。方法收集各类高危因素的新生儿脐动静脉血共98例,分为五组:生后窒息组、羊水Ⅲ度浊组、母有高血压病史组、宫内发育迟缓组、母亲患糖尿病组;另采集正常足月儿组(对照组)31例脐动静脉血,测定血EPO、ET水平及血pH、PCO2、PO2、BE水平。结果生后窒息组、羊水Ⅲ度浊组、母有高血压病史组、宫内发育迟缓组、母亲患糖尿病组是影响脐血EPO水平的重要因素(P<0.01),而窒息组是影响脐血ET的重要因素,结果示:脐血EPO可以作为胎儿急慢性缺氧的一个重要指标,ET可以作为胎儿急性缺氧的一个重要指标,分析窒息组、羊水混浊组pH、PCO2、PO2、BE水平,结果窒息组和羊水混浊组的血pH、PCO2、PO2、BE和对照组相比有显著性差异,而血窒息组EPO、ET水平和血pH、PO2呈明显的负相关,P<0.001有统计学意义。结论脐血EPO、ET、pH、PO2可以作为围产期危险因素的可靠、灵敏的指标。
Objectives We examine cord blood levels in newborns with different types of newborns at birth to determine whether erythropoietin and endothelin can be used as more reliable and sensitive laboratory markers for risk factors for perinatal newborn infants. Methods A total of 98 umbilical arteriovenous blood samples were collected for neonates with various risk factors and divided into five groups: postnatal asphyxia group, amniotic fluid Ⅲ degree turbidity group, mother with hypertension history group, intrauterine growth retardation group and maternal diabetes group In addition, 31 cases of umbilical arterial and venous blood were collected from normal full-term infants (control group). The levels of EPO and ET, blood pH, PCO2, PO2 and BE were measured. Results Asphyxia group, amniotic fluid Ⅲ degree turbidity group, mother history of hypertension group, intrauterine growth retardation group and maternal diabetes group were the important factors affecting umbilical cord blood EPO level (P <0.01) The results showed that: umbilical cord blood EPO can be used as an important indicator of fetal acute and chronic hypoxia, ET can be used as an important indicator of fetal acute hypoxia, asphyxia group, amniotic fluid opacity group pH, PCO2, PO2, BE Level, the results of asphyxia group and amniotic fluid opacity group of blood pH, PCO2, PO2, BE compared with the control group, there was a significant difference, while the blood asphyxia group EPO, ET levels and blood pH, PO2 showed a significant negative correlation, P <0.001 There is statistical significance. Conclusion Umbilical cord blood EPO, ET, pH, PO2 can be used as reliable and sensitive indicators of perinatal risk factors.
