应用单克隆抗体B72.3和免疫酶标方法检测了肿瘤相关糖蛋白TAG-72在110例胃癌,36例肠型不典型增生和303例肠化组织中的表达。根据粘液染色将肠化分为3型:Ⅰ型为完全型,Ⅱ和Ⅲ型为不完全型,Ⅱ、Ⅲ两型的区别在于后者柱状粘液细胞含有硫酸粘液,而前者则无硫酸粘液。胃癌TAG-72染色阳性率为60.9%。肠型不典型增生及癌旁肠化组TAG-72阳性率显著高于其它良性病变肠化组(P<0.05~0.01);Ⅲ型肠化TAG-72阳性率显著高于Ⅰ、Ⅱ型肠化(P<0.01),提示Ⅲ型肠化与胃癌发生有关。结果表明,检测胃病组织TAG-72有助于印戒细胞癌的诊断和鉴别诊断,对确定胃癌高危人群,提高早癌的检出率亦可能有重要意义。
The expression of tumor-associated glycoprotein TAG-72 was detected in 110 cases of gastric cancer, 36 cases of intestinal dysplasia and 303 cases of intestinal metaplasia using monoclonal antibody B72.3 and immunoenzymatic method. According to the mucin staining, the intestinal metaplasia was divided into three types: type I is complete, type II and type III are incomplete, and the difference between type II and type III is that the latter columnar mucinous cells contain sulphuric acid mucus, while the former has no sulphuric acid mucus. The positive rate of TAG-72 staining in gastric cancer was 60.9%. The positive rate of TAG-72 in intestinal dysplasia and para-carcinoma adjacent to intestinal metaplasia was significantly higher than that in other benign lesions (P<0.05-0.01). The positive rate of TAG-72 in type III intestinal metaplasia was significantly higher than that of type I and II intestine. (P<0.01), suggesting that type III intestinal metaplasia is related to the occurrence of gastric cancer. The results showed that the detection of gastric tissue TAG-72 is helpful for the diagnosis and differential diagnosis of signet-ring cell carcinoma. It may also be of great significance to determine the high risk population of gastric cancer and improve the detection rate of early cancer.
