目的探讨超声联合羊水相关激素评价胎儿甲状腺功能的可行性。方法测量172例孕18~41周(GA)正常单胎胎儿的羊水甲状腺激素(T3、FT3、TSH、T4、FT4)浓度并建立正常值。对可疑甲状腺疾病高危胎儿行超声筛查,发现甲状腺增大时再行羊膜腔穿刺行羊水激素测定。结果羊水FT3、T3浓度与孕周有相关性,回归方程:yFT3=0.051GA-0.551,yT3=0.015GA+0.799(P均<0.05);羊水FT4、T4、TSH浓度与孕周无明显相关性。超声联合羊水激素共筛查4例胎儿甲减。结论超声联合羊水甲状腺激素测定对评价胎儿甲状腺功能有较大的临床应用价值。
Objective To investigate the feasibility of ultrasound combined with amniotic fluid hormone to evaluate fetal thyroid function. Methods The levels of amniotic fluid thyroid hormones (T3, FT3, TSH, T4, FT4) in 172 singleton fetuses with gestational age of 18 to 41 weeks (GA) were measured and normal values were established. Suspected thyroid disease in high-risk fetus ultrasound screening, thyroid enlargement found amniocentesis when amniotic fluid hormone determination. Results The correlation between the concentration of amniotic fluid FT3 and T3 was correlated with gestational age. The regression equation was: yFT3 = 0.051GA-0.551, yT3 = 0.015GA + 0.799 (all P <0.05). There was no significant correlation between FT4, . 4 cases of fetal hypothyroidism were screened by ultrasound and amniotic fluid hormone. Conclusion Ultrasound combined with amniotic fluid thyroid hormone assay for the evaluation of fetal thyroid function have greater clinical value.
