目的探讨非糖尿病与糖尿病急性冠脉综合征患者肌钙蛋白T(cTnT)的变化及预后分析。方法 2013年8月到2015年2月选择在我院进行诊治的急性冠脉综合征(ACS)患者作为研究对象,检测入院时血清cTnT及术后30d血清cTnT的含量。急性冠脉综合征患者都给予经皮冠状动脉介入治疗,随访术后30天冠脉不良事件。结果糖尿病ACS和非糖尿病ACS患者各94例入组进行分析。非糖尿病ACS组患者的血清cTnT含量显著低于糖尿病ACS组(P<0.05)。两组患者术后30d血清cTnT含量都明显低于入院时(P<0.05),同时非糖尿病ACS组患者的术后30d血清cTnT含量低于糖尿病ACS组(P<0.05)。随访3个月,非糖尿病ACS组患者的主要冠脉不良事件发生率明显少于糖尿病ACS组(P<0.05)。直线相关性分析两组患者的预后主要冠脉不良事件都与入院时cTnT含量存在明显的相关性(P<0.05)。结论非糖尿病急性冠脉综合征患者与糖尿病急性冠脉综合征患者均伴随有cTnT表达的上升,介入治疗能降低cTnT的表达,且预后主要冠脉不良事件的发生与入院时cTnT表达存在明显相关性。非糖尿病ACS患者介入术前cTnT含量和术后主要冠脉不良事件发生率均低于糖尿病ACS患者。
Objective To investigate the changes and prognosis of troponin T (cTnT) in non-diabetic and diabetic patients with acute coronary syndrome. Methods From August 2013 to February 2015, patients with acute coronary syndrome (ACS) who were selected for diagnosis and treatment in our hospital were selected as study subjects. Serum cTnT levels at admission and cTnT levels at 30 days were measured. Patients with acute coronary syndrome were treated with percutaneous coronary intervention and were followed up for adverse coronary events 30 days after surgery. Results 94 cases of diabetic ACS and non-diabetic ACS were enrolled in the study. Serum cTnT levels were significantly lower in non-diabetic ACS patients than in diabetic ACS patients (P <0.05). The levels of serum cTnT in both groups were significantly lower than those on admission (P <0.05) 30 d after operation, while the levels of cTnT in serum of non-diabetic ACS group were lower than those of diabetic ACS group 30 d after operation (P <0.05). At 3 months follow-up, the incidence of major adverse coronary events in non-diabetic ACS patients was significantly less than that in diabetic ACS patients (P <0.05). Linear correlation analysis showed that there was a significant correlation between the prognosis of major coronary events and the content of cTnT on admission (P <0.05). Conclusions Patients with nondiabetic patients with acute coronary syndrome and patients with acute coronary syndromes are associated with increased cTnT expression. Interventional therapy can reduce the expression of cTnT, and the prognosis of major coronary events is significantly correlated with the expression of cTnT at admission Sex. The preoperative cTnT levels in patients with non-diabetic ACS and postoperative major adverse coronary events were lower than those in patients with diabetic ACS.
