心房内3、4相传导阻滞为间歇性不完全性房内传导阻滞的一种特殊类型。现报告右房内3相阻滞、左房内4相阻滞各1例。例1 患者男性,21岁,临床诊断:风湿性心脏病、二尖瓣狭窄伴关闭不全。心电图(图1)Ⅱa、Ⅱb、Ⅱc系同时不连续记录。Ⅱa示两种P波形态:(1)时限0.10s、电压0.15mV,呈双峰形。峰距0.04s,为“二尖瓣型P波”,其P-P间期0.60—0.67s;(2)时限0.09—0.10s、电压0.32—0.35mV呈“肺型P波”,其P-P间期0.60—0.69s,符合间歇性不完全性房内传导阻滞。
Atrial 3,4-phase conduction block is intermittent incomplete atrial conduction block a special type. Now report the right atrium 3-phase block, left atrial block 4 in 1 case. Case 1 male patient, 21 years old, clinical diagnosis: rheumatic heart disease, mitral stenosis with incomplete closure. ECG (Figure 1) Ⅱ a, Ⅱ b, Ⅱ c Department at the same time not continuous recording. Ⅱ a shows two kinds of P wave morphology: (1) the time limit 0.10s, voltage 0.15mV, was bimodal. Peak distance 0.04s, “mitral-type P wave”, the PP interval of 0.60-0.67s; (2) the time limit of 0.09-0.10s, voltage 0.32-0.35mV was “pulmonary P wave”, the PP interval 0.60-0.69s, in line with intermittent incomplete room conduction block.
