目的通过胎儿畸形的超声筛查尤其是Ⅲ级筛查,探讨超声检测胎儿肢体末端结构的二、三维成像的比较研究。方法从日常工作的超声Ⅲ级筛查的病例中针对孕18~24周胎儿Ⅲ级筛查超声发现有胎儿肢体末端结构异常的病例实施进一步的二、三维成像。结果对发现有胎儿畸形的病例进行跟踪随访,从引产及出生的病例中得到有胎儿肢体末端结构异常及姿势异常的20例、23处畸形,回顾分析胎儿二维声像图诊断符合率15/23(65.2%),三维声像图诊断符合率21/23(91.3%)。结论在胎儿超声Ⅲ级筛查中虽然未要求对指趾异常的筛查,但通过对指趾的三维成像可以进一步明确胎儿肢体末端的结构异常,从而提高检出率。在临床上的推广可以给更多的家庭带来福祉。
OBJECTIVE: To compare the two-dimensional and three-dimensional imaging of fetal limbs by ultrasound in the screening of fetal malformations, especially in stage III screening. Methods From the daily work of ultrasound Ⅲ screening cases for pregnant 18 to 24 weeks fetus Ⅲ level screening ultrasound found that the structure of the fetal limb abnormalities in the implementation of further two-dimensional imaging. Results The cases of fetal malformations were followed up. Twenty cases with abnormal limb posture and abnormal posture were obtained from the induced abortion and the born cases. Twenty-three deformities were retrospectively analyzed. The coincidence rate of fetal two-dimensional sonography was 15 / 23 (65.2%), three-dimensional sonography diagnostic coincidence rate of 21/23 (91.3%). Conclusion Although there is no requirement to screen for abnormal finger toe in grade Ⅲ screening of fetal ultrasound, the abnormality of the end of fetal limb can be further clarified by three-dimensional imaging of the digit to increase the detection rate. In the clinical promotion can bring more family well-being.
