Infringement of the inferior vena cava caused by adrenocortical carcinoma is rare, and only 6 cases have been reported in the literature. Usually confirmed by autopsy, the authors reported 1 as follows: female patient, 28 years old, abdominal distention, right lower back pain and calf edema for 1 month. Physical examination: ascites, hepatomegaly, upper abdomen and lumbar varices with sag edema, without masculinity. ALT 1,380 IU (normal 10 ~ 40 IU), AST680 IU (normal 10 ~ 40 IU), prothrombin time 18 seconds (normal 12 seconds), chest X-ray is normal. Ultrasound showed right adrenal tumors and the inferior vena cava segment was obstructed to the right atrium level.
