目的:探讨关节镜下穿骨隧道修补手术治疗肩袖撕裂的效果及对患者肩关节功能和生活质量的影响。方法:选择浙江省荣军医院2016年1月至2019年1月收治的肩袖撕裂患者31例为研究对象，均行关节镜下穿骨隧道修补手术治疗。观察手术指标，术前、术后6个月和术后12个月视觉模拟评分法（VAS评分）、Constant-Murley肩关节评分和洛杉矶加福尼亚大学评分（UCLA），及术前和术后12个月生活质量（SF-36）量表评分变化。结果:患者均完成手术，且全部得到随访，术后无神经损伤、伤口感染及切口血肿等并发症。VAS评分术后6个月［（3.25±0.76）分］和术后12个月［（1.63±0.53）分］低于术前［（7.12±1.08）分］（n t=18.764、29.220，均n P<0.05）；且术后12个月低于术后6个月（n t=11.195，n P<0.05）。Constant-Murley肩关节评分术后6个月［（58.97±8.10）分］和术后12个月［（83.42±4.19）分］高于术前［（38.41±5.46）分］（n t=13.477、41.876，均n P<0.05）；且术后12个月高于术后6个月（n t=17.167，n P<0.05）。UCLA评分术后6个月［（25.43±3.65）分］和术后12个月［（33.71±5.46）分］高于术前［（17.96±2.51）分］（n t=10.798、16.782，均n P<0.05）；且术后12个月高于术后6个月（n t=8.073，n P<0.05）。术后12个月生理机能［（82.19±6.59）分］、社会功能［（75.34±7.23）分］、躯体功能［（72.19±5.40）分］、精神健康［（78.91±7.32）分］及一般健康状态［（76.29±7.29）分］均高于术前［（45.83±5.42）分、（41.92±6.18）分、（35.83±6.41）分、（47.29±6.47）分、（44.36±6.22）分］（n t=27.286、22.499、27.778、20.724、21.335，均n P<0.05）。n 结论:关节镜下穿骨隧道修补手术治疗肩袖撕裂患者效果良好，可改善患者肩关节功能和生活质量。“,”Objective:To investigate the effect of arthroscopically assisted bone tunnel drilling in the repair of rotator cuff tears and its effect on shoulder joint function and quality of life.Methods:A total of 31 patients with rotator cuff tears who received treatment in Rongjun Hospital of Zhejiang Province from January 2016 to January 2019 were included in this study. All of them underwent arthroscopically assisted bone tunnel drilling for repair of rotator cuff tears. Operation indexes were observed. Visual Analogue Score, Constant-Murley shoulder score, and University of California Los Angeles (UCLA) shoulder score before and 6 and 12 months after surgery were determined. The MOS 36-Item Short Form Health Survey (SF-36) score before and 12 months after surgery was determined.Results:All patients completed the surgery and were followed up. None of them had postoperative complications such as nerve injury, wound infection and incision hematoma. The VAS score at 6 months [(3.25 ± 0.76) points] and 12 months after surgery [(1.63 ± 0.53) points] was significantly lower than that before surgery [(7.12 ± 1.08) points,n t = 18.764, 29.220, both n P < 0.05], and the VAS score at 12 months after surgery was significantly lower than that at 6 months after surgery ( n t = 11.195, n P < 0.05). The Constant-Murley shoulder score at 6 months [(58.97 ± 8.10) points] and 12 months after surgery [(83.42 ± 4.19) points] was significantly higher than that before surgery [(38.41 ± 5.46) points] ( n t = 13.477, 41.876, both n P < 0.05). The Constant-Murley shoulder score at 12 months after surgery was significantly higher than that at 6 months after surgery ( n t = 17.167, n P < 0.05). The UCLA score at 6 months [(25.43 ± 3.65) points] and 12 months after surgery[(33.71 ± 5.46) points] was significantly higher than that before surgery [(17.96 ± 2.51) points, n t = 10.798, 16.782, both n P < 0.05]. The UCLA score at 12 months after surgery was significantly higher than that at 6 months after surgery ( n t = 8.073, n P < 0.05). The physical functioning [(82.19 ± 6.59) points], social functioning [(75.34 ± 7.23) points], role-physical functioning [(72.19 ± 5.40) points], mental health [(78.91 ± 7.32) points] and general health scores [(76.29 ± 7.29) points] at 12 months after surgery were significantly higher than those before surgery [(45.83 ± 5.42) points, (41.92 ± 6.18) points, (35.83 ± 6.41) points, (47.29 ± 6.47) points, (44.36 ± 6.22) points; n t = 27.286, 22.499, 27.778, 20.724, 21.335, all n P < 0.05).n Conclusion:Arthroscopically assisted bone tunnel drilling for repair of rotator cuff tears is effective, which can improve shoulder joint function and quality of life.
