目的:探讨肿瘤相关抗原CA15-3在良性与恶性乳腺病变及乳腺转移癌和非乳腺转移癌组织中的表达和意义。方法:应用免疫组织化学SP法检测85例乳腺癌、21例良性乳腺病变、14例乳腺癌转移、17例胃癌转移和3例鼻咽癌转移组织中的CA15-3的表达情况。结果:85例乳腺癌中CA15-3阳性表达者为92.9%,其中癌细胞胞浆阳性占92.4%,胞膜阳性占7.6%;21例良性乳腺病变阳性表达为90.5%,其中胞浆阳性占5.3%,胞膜阳性占94.7%,两者之间差异有显著性(P<0.01)。14例乳腺癌转移CA15-3阳性表达为100%,其中胞浆阳性占71.4%,胞膜阳性占28.6%(P<0.05);17例胃癌转移阳性为82.4%,其中胞浆阳性占14.3%,胞膜阳性占85.7%(P<0.01)。3例鼻咽癌转移均为阴性。结论:CA15-3胞浆阳性可作为判断良恶性乳腺病变以及乳腺癌淋巴结转移和非乳腺癌淋巴结转移的一种指标
Objective: To investigate the expression and significance of tumor-associated antigen CA15-3 in benign and malignant breast lesions, breast metastases and non-breast metastases. METHODS: Immunohistochemical SP method was used to detect the expression of CA15-3 in 85 cases of breast cancer, 21 cases of benign breast lesions, 14 cases of breast cancer metastasis, 17 cases of metastasis of gastric cancer and 3 cases of nasopharyngeal carcinoma metastasis. Results: The positive expression rate of CA15-3 in 92 cases of breast cancer was 92.9%, of which 92.4% were positive for cytoplasm and 7.6% were positive for membrance, and 21 cases were positive for benign breast lesions. 5%, of which cytoplasm positive 5.3%, membrane-positive 94.7%, the difference between the two was significant (P <0.01). The positive expression of CA15-3 in 14 cases of breast cancer metastasis was 100%, of which 71.4% were cytoplasmic positive, 28.6% were membranous positive (P<0.05), and the positive rate of metastasis in 17 cases was 82.4%. Among them, cytoplasm positive was 14.3%, and membrane positive was 85.7% (P<0.01). 3 cases of metastasis of nasopharyngeal carcinoma were negative. Conclusion: The cytoplasmic positive for CA15-3 can be used as an index to determine benign and malignant breast lesions, lymph node metastasis of breast cancer and lymph node metastasis of non-breast cancer.
