目的观察脐血白细胞介素-10(IL-10)、铅水平与缺氧缺血性脑病(HIE)新生儿行为神经测定(NBNA)的关系。方法将57例中重度HIE新生儿分为低血铅组(<60μg/L)37例及高血铅组(≥60μg/L)20例,2组分别进行脐血IL-10、铅检测及NBNA评分,观察脐血IL-10和铅水平与NBNA的关系,同时以健康新生儿28例作为健康对照组。结果 HIE组与健康对照组脐血铅含量差异无统计学意义(P>0.05);HIE组脐血IL-10水平显著高于健康对照组(P<0.01);HIE高血铅组与HIE低血铅组IL-10水平差异无统计学意义(P>0.05);HIE高血铅组比HIE低血铅组NBNA评分显著降低(P<0.05);HIE高血铅组和HIE低血铅组IL-10水平和NBNA评分均呈负相关(r分别为-0.819、-0.714,P均小于0.05)。结论提示IL-10可监测HIE患儿病情,判断预后,脐血铅含量增高可对婴儿神经行为发育产生不良影响,影响HIE患儿的预后。
Objective To investigate the relationship between cord blood interleukin-10 (IL-10), lead levels and behavioral neurological assessment (NBNA) in neonates with hypoxic-ischemic encephalopathy (HIE). Methods Fifty-seven neonates with moderate-to-severe HIE were divided into two groups: low blood lead group (<60μg / L), high blood lead group (≥60μg / L) and 20 cases. NBNA score to observe the relationship between cord blood IL-10 and lead levels and NBNA, while healthy newborns 28 cases as a healthy control group. Results There was no significant difference in cord blood lead levels between HIE group and healthy control group (P> 0.05). The level of IL-10 in cord blood of HIE group was significantly higher than that of healthy control group (P <0.01) The level of IL-10 in blood lead group had no significant difference (P> 0.05). The NBNA score in HIE hyperbilirubinemia group was significantly lower than that in HIE low blood lead group (P <0.05); HIE high blood lead group and HIE low blood lead group The levels of IL-10 and NBNA were negatively correlated (r = -0.819, -0.714, P <0.05 respectively). Conclusions IL-10 can monitor the condition of children with HIE and determine the prognosis. The increase of cord blood lead levels can have adverse effects on the neurobehavioral development of infants and affect the prognosis of children with HIE.
