目的探讨64排螺旋CT三维成像技术在颅底肿瘤外科中的应用价值。方法颅底肿瘤患者109例,包括脑膜瘤40例,神经鞘瘤18例,动脉瘤17例,胆脂瘤8例,垂体瘤22例,血管母细胞瘤4例,术前64排CT三维重建,以容积再现法(VR)及最大面积投影法(MIP)等作图像三维重建处理,模拟手术入路。结果64排螺旋CT三维成像清楚显示了颅底肿瘤的生长部位、形态、大小及其与大血管、颅骨的三维关系,与手术及病理检查吻合。结论64排螺旋CT三维重建可清楚显示肿瘤、血管及其与邻近组织的关系,对颅底肿瘤诊断及鉴别诊断、设计手术入路及方法有着重要价值。
Objective To investigate the value of 64-slice spiral CT three-dimensional imaging in the surgical treatment of skull base tumors. Methods 109 cases of skull base tumor patients including 40 cases of meningiomas, schwannoma in 18 cases, 17 cases of aneurysms, 8 cases of cholesteatoma, pituitary tumors in 22 cases, 4 cases of hemangioblastoma, 64-slice CT three-dimensional reconstruction , Volume rendering method (VR) and maximum area projection method (MIP) for three-dimensional reconstruction of images, simulation of surgical approach. Results 64-slice spiral CT three-dimensional imaging clearly shows the growth of the skull base tumor, shape, size and its relationship with the large vessels, skull three-dimensional, and surgical and pathological examination anastomosis. Conclusion Three-dimensional reconstruction of 64-slice spiral CT can clearly show the relationship between tumor, blood vessels and adjacent tissues. It is of great value in the diagnosis and differential diagnosis of skull base tumors and the design of surgical approaches and methods.
