目的了解湖北省武汉市社区医务人员对高血压管理的参与及满意度现状及其满意度影响因素,为进一步完善国内社区高血压管理提供可行性建议。方法于2015年1月在武汉市随机抽取14个行政区249名社区医务人员进行问卷调查。结果武汉市249名社区医务人员中,有71.9%的人认为高血压管理质量基本达标,有40.2%的人对高血压管理工作整体表示满意;有60.6%的社区医务人员将高血压临床诊疗视为本职工作,但社区医务人员在护理服务、健康档案管理和信息系统维护方面的参与度较低,分别占17.3%、28.1%、21.3%;在社区高血压管理工作改进评价方面,要求改进医疗保险对慢性病支持、患者参与配合度、员工工作激励和经费支持的比例较高,分别占67.1%、69.5%、71.9%、73.9%;多因素非条件logistic回归分析结果显示,对培训工作、药品品种、对象配合和随访质量满意的武汉市社区医务人员对高血压管理的满意度越高。结论武汉市社区医务人员对社区高血压管理工作满意度一般,参与度有待提高;对高血压管理培训工作、药品品种、对象配合和随访质量的满意情况是社区医务人员对高血压管理满意度的主要影响因素。
Objective To understand the current situation of participation and satisfaction of hypertension and community health workers in community health centers in Wuhan City, Hubei Province and their influencing factors, so as to provide feasible suggestions for further improving the management of hypertension in communities in China. Methods In January 2015, 249 community health workers from 14 administrative districts were randomly selected in Wuhan for questionnaire survey. Results Among the 249 community health workers in Wuhan, 71.9% believed that the quality of hypertension management basically met the standard, 40.2% of them were satisfied with the overall management of hypertension. 60.6% As their own jobs, but the participation of community health workers in nursing services, health records management and information system maintenance was relatively low, accounting for 17.3%, 28.1% and 21.3% respectively; and the improvement of community-based hypertension management evaluation required improvement of medical services Insurance accounted for 67.1%, 69.5%, 71.9% and 73.9% of patients with chronic diseases support, patient engagement, employee motivation and financial support. Multivariate non-conditional logistic regression analysis showed that the training, medicine Variety, object coordination and follow-up quality of community health workers in Wuhan City, the higher the satisfaction of hypertension management. Conclusion The community health workers in Wuhan have satisfied with the management of community hypertension and their participation needs to be improved. The satisfaction with the management of hypertension, the variety of drug, the cooperation with the target and the quality of follow-up are the satisfaction of community medical staff in hypertension management the main factor of influence.
